COVID-19 Vaccination Event - 12 to 15 year old children @ PHS
Good evening,
You are receiving this message because you expressed interest in your 12 to 15 year old child(ren) participating in a COVID-19 vaccination event.
On May 23, 2021 and June 13, 2021 from 10 a.m. to 5 p.m. at PHS East we will be holding a vaccination event. We are still awaiting notice that Rite Aid will be able to administer the vaccine on these dates to students aged 12 to 15. We are proceeding with planning the event because we remain very hopeful that the approval will arrive in time.
This event is for the two-dose Pfizer vaccine for students aged 12 or older as of May 23, 2021. Rite Aid Pharmacy is providing and administering the vaccines. Participating students must have a parent or guardian present.
To select a time slot for this event, please complete the form HERE. If you no longer wish to participate in this event, there is a place to indicate that on the form.
Questions about the event can be submitted here.
We look forward to hearing from you!
