A Message from the School Nurse

SCHOOL PHYSICALS AND IMMUNIZATIONS:

Although District 97 is starting the 2020-2021 school year with remote learning, the State of Illinois requires students entering kindergarten, as well as students new to the State of Illinois schools in any grade to turn in a current school physical (Certificate of Child Health Examination and Immunization record) with up-to-date immunizations (immunization requirements for the State of Illinois can be found here). The health examination must have been done within one year of the first day of school.


If your child is transferring from another Illinois school, transferred health examination records will be accepted as long as they are received by the deadline and found to be complete and in compliance. This physical exam must be recorded on the State of Illinois Certificate of Child Health Examination form.


If you have not already provided these health records when you registered your child, please provide your child's records directly to our school nurse at this time. You may provide them by fax (Lincoln fax: 708-524-3124) or by scanning and emailing a complete legible copy to our school nurse (gdonovan@op97.org). You may also choose to drop-off your child's health records to the Lincoln School administrative assistant on school days (between 9 am and 3 pm).


Please note: The Certificate of Child Health Examination records and records of all required immunizations must be provided before October 15, 2020 to avoid exclusion from school.



VISION EXAMINATIONS:

Kindergarten students and students who enter the Illinois school system for the first time Illinois are required to have an eye examination by an optometrist or ophthalmologist, by October 15, 2020. The examination must have been completed within 12 months prior to the October 15th deadline.

Students who are unable to obtain a vision examination due to undue burden or lack of access must complete the State of Illinois Eye Examination Waiver Form which can be found on our website, under Health Services (www.op97.org).


DENTAL EXAMINATIONS:

Students in kindergarten and 2nd grade, or any new student new to Illinois schools, are required to have a complete dental examination by May 15, 2021. The examination must have been completed within 18 months prior to the May 15th deadline.

Students who are unable to obtain a dental examination due to undue burden or lack of access must complete the State of Illinois Dental Examination Waiver Form which can be found on our website, under Health Services (www.op97.org).

District 97 Information

Continued Social-Emotional Wellness Check-Ins for Students
District 97 school psychologists and nurses have collaborated to offer access to virtual mental health wellness checks at each school this fall. Parents may sign up children for 25-minute Telehealth sessions where they can access an open ear for their sources of stress and learn self-care and coping strategies. Students may attend up to three sessions per trimester and may then be referred for other school-based supports if needed.

If your child is experiencing an immediate mental health crisis, please do not use this service and instead call the Thrive 24-Hour Crisis Hotline at 708-383-7500. You may also refer to this local resource guide created by the Community Mental Health Board of the Oak Park Township.


Next Meal D97 Meal Distribution: Tuesday, Sept. 15
During Trimester 1, meals will be provided free of cost to any District 97 student in need. Meal distributions will be held every Tuesday, from 10:30 a.m. to 1 p.m., at Longfellow Elementary School (for Julian and feeder schools), 715 S. Highland Ave., and Brooks Middle School (for Brooks and feeder schools), 325 S. Kenilworth Ave.

Each bag of food will contain five breakfasts and five lunches. Click here for the September 2020 menus.

D97 to Host Virtual Parent Circles on September 16 and 23
District 97 families are invited to join two upcoming Virtual Parent Circles, which will be facilitated in partnership with the E-Team of Oak Park. Events will be held:

These sessions were organized to accommodate families who were unable to participate in parent circles during our Aug. 13 Parent University. The sessions are designed to build community among District 97 parents and guardians and provide support as we navigate remote learning.

Little Lions Read!

We understand that these are unprecedented times and families may be in need of support as they build remote learning libraries! The Lincoln School PTO is generously offering to purchase books for students who need books for remote learning. Your response will be shared with the Lincoln front office staff and once the books are ordered, they will be available for pick up at school or delivery to your home.
