In order to monitor the status of COVID-19 effects on our staffing, we need each school to account DAILY for any Auxiliary Staff (those folks you have designated to cover classes in the event that a substitute teacher is not available) that are used. Please designate someone to complete the form HERE EVERY DAY.
ONLY ONE FORM PER DAY SHOULD BE COMPLETED BY YOUR SCHOOL!
COVID-19 Information
What do I do if an employee reports symptoms of COVID-19 or shares information about exposure to someone who is sick? Employee reports to supervisor that he/she is experiencing a symptom or has been exposed to someone who is experiencing a symptom. Supervisor completes the following script with the employee: Employee COVID-19 Supervisor Script. (If you complete this form with an employee who is not currently experiencing symptoms of COVID-19 and he/she later tests positive or begins to exhibit symptoms of COVID-19, you must complete the form again at that point.) Forms submitted in step two will automatically notify the appropriate members of our HR team, who will reach out to the employee and supervisor regarding next steps. If an employee is excluded from work, FMLA and Employee Information Specialist Jennifer Berry will contact the employee to discuss appropriate leave options and the required return to work documents or process for the employee. If an employee is excluded from work for quarantine but not sick, Human Resources Coordinator Meredith Seibert will collaborate with the supervisor to determine telework eligibility. If the employee is eligible for telework, Meredith will share the Telework Accountability Form with the supervisor for the employee and supervisor to complete. The form must be completed, signed, scanned and returned to HR before the employee can begin teleworking. Do not communicate to an employee that he/she may telework unless you have approval from Human Resources. All employees must work with Jennifer Berry before returning to work following a COVID-19 work exclusion. Step 4.
