Objective
The transitioning planning team will help start Kim's educational journey off on the right path and to ensure her transition to Kindergarten goes smooth and positively as much as possible but also for you, her family members as well. The team will work collaborative with Kim's family members and her teachers and care professionals to provide the necessary support to meet her individualize needs and concerns in order for her to have a successful school year and achieve the goals that are set out for her.

The Transition Process
The transition meeting bridges home and the education system together to ensure you, Kim's parents are involved in the process of Kim’s education and provide adequate time for her and you to learn and practise the skills and routines for a smooth transition. The goal is to help Kim adjust quickly to the school setting, enjoy learning, and develop a positive attitude towards education. During the process, Kim's strengths and needs will be addressed, along with the specific goals to achieve it and short and long term action plans for her. But also, collaborating and sharing information among key members such as health care professionals, to help implement and carry out the transition.
What will happen during and after the meeting?
During the meeting, Kim's needs, strength, and goals are identify and the appropriate members of Kim should be included in all the phases of the development, implementation, and evaluation of the transition plan.
To make Kim feel more comfortable with the process, parents and family members can help Kim communicate with other members of the planning team, suggest options that will meet Kim's needs and assisting in carryout the actions assigned to Kim in the transition plan
How will the transition plan help prepare for the future?
It is important for you, Kim's parents to register her early to allow the principal to met with you and Kim to determine if there are special needs or issue e.g. health, that need to be addressed. It will also allow you to develop a positive relationship with the school and to address concerns and needs of their child. Also, you may schedule a classroom visit to allow Kim to become familiar and comfortable in her new classroom and meet her teachers.
