Exemption Information

Texas law allows for an exemption from immunizations for reasons of conscience, including a religious belief. To claim an exemption due to reasons of conscience, a student's parent or legal guardian must request, sign, and submit an official DSHS affidavit form to the child's school. EMS ISD Back to School Immunization Clinics

Passport Health/Staying Healthy Medical Services will be conducting a children’s vaccination clinic to get your student attending Eagle Mountain-Saginaw ISD up to date for the school year.


The following forms of payment will be accepted:


Medicaid/Chip - No charge for vaccines. Must provide a paper photocopy of card


Aetna, Blue Cross Blue Shield, Cigna, Humana and United Health Care PPO plans - A paper photocopy of insurance card and driver's license must be provided to receive vaccines at no charge.


Uninsured - $10 per vaccine ($40 maximum per child) Cash or Check Only


Vaccines Available:


Chicken Pox, Dtap, Hepatitis A, Hepatitis B, HIB, HV, Meningococcal, MMR, Polio, Pneumonia, and Tdap


A printed current vaccine record is required, and a Parent or Guardian must be present

Immunization Collaboration of Tarrant County will be hosting Immunization Events during the month of August.
