Chaperone Assignments for All School Field Trip
Chaperone Assignment
Chaperones for each group have been selected. Confirmations went home today to parents. Click on the link below to see your assigned chaperones. You will notice that some groups have 3 parents based on the needs of the group. You will also note that some groups have also been assigned WHS Support Staff members.
We worked hard to make sure that each of the groups was adequately supported. Please be sure to see me if you have any questions or concerns regarding your assigned chaperones.
Brookfield Zoo Permission Slips
Field Trip Permission Forms
Classroom teachers - please keep track of the general permission slip for your students. Please monitor and hold all students accountable for turning it in. Once you ENTIRE class has their forms turned in, please turn them in to the office. Thank you for you support and cooperation with this.
FIELD TRIP DETAILS COMING SOON
- Assigned Chaperones will come to school with their children on Tuesday, May 21st. They will sign in the office and then go with their child to their regular grade level teacher.
- Once attendance is complete, Dave will make the announcement for us to get into our Star Constellation Groups. Parent chaperones will travel with their child to their assigned group.
- Parent Chaperones will ride the bus with their assigned group.
- Each group will need some sort of lunch "box" to house the sack lunches for the students, parent chaperones, and group leader
- Details about how lunch will work will be coming soon.
- Each group leader will receive a folder with their group pictures, a zoo map, important phone numbers, and a schedule for the day.
- Targeted departure time from Hammerschmidt is 9:00 a.m.
- Targeted arrival time back at Hammerschmidt is 1:30 p.m.
- More information will be coming soon.
Ducks Fly Together!
The Ducks Video Planning Mtg. will take place on Wednesday, 5/15 at 3:30 p.m. Feel free to join us if interested.
Club Sponsors and Awards-- CERTIFICATE DUE DATE- Friday, May 17th
These certificates will either be presented at an informal grade level ceremony, or individually by Dave visiting classrooms to present them to students.

Madison and her family had so many positive things to say about her time at Hammerschmidt. Seeing her in her tiara serves as a great reminder to us all that we never know what our current students will become!

Jenn Kurtz
Family:
I am married and have two daughters, Jaelyn (age 4) and Ariana (age 1). We currently live in Lombard. I grew up in Oak Park and my parents still live there. My brother, sister-in-law, niece, and nephew now live in Oak Park as well.
We made some long overdue improvements to our house and yard.
College:
I worked at a bakery for over 7 years in high school and part of college.
And Now...
Lisa Flaishaker
Immediate family: Jim Flaishaker, Julia Flaishaker (15 sophomore), Brooklyn Flaishaker (12- 7th grade), Zoey (8 years old- French Bulldog and Boston Terrier mix) I have four older brothers and a twin sister. My brothers all live near by. My sister lives in California.
I went to the gym every day possible. I read four fifth grade novels and three adult novels.
College:
Northern Illinois University - Roosevelt University (for master's degree)
