Just a quick reminder our Senior T-Shirt Design Contest will be closing Monday, 10/11 at 10pm.

To be considered for a submission you must:

• Complete the attached form
• Your name and personal email address must be accurately submitted
• Note: Any forms erroneously submitted, will not be entered into the contest
• Once you submit an entry, you will receive a confirmation email from me, Amy Edwards

Please note: If you have already submitted a design but did not complete the form with accurate information, you may re-submit your design to be considered for this contest. If you have submitted a design but have not received a confirmation email, please email me with your information to ensure your design is entered.
