Parker County Hospital District will be coming to each campus on Thursday, September 20th, 2018, to give Flu Vaccinations to our students and staff.
As in the past, students will be free. For staff the cost is $20.00 cash/check or bring a copy of your insurance card for flu vaccines and they will file with your insurance if it covers vaccines.
PCHD will be at each campus starting at the Elementary campus and finishing up at the Jr. High Campus. Times are estimated below..... For staff, feel free to come to any of the campuses.
Parents/guardians of students under 18 years of age will need to complete the consent form and answer the questionnaire. PCHD will not give the flu vaccination without signed and completed consent form. There are several ways to get a consent form: 1) on our Brock ISD website under Departments then Health Services, Flu Consents 2018-2019 2) forms will be sent home with Elementary, Intermediate and Jr. High Students or 3) in the front offices of those campuses.
High School students may pick up a form at the front office or download off the website.
Below are the estimated times for each campus.
8:00am -9:30 am Elementary Campus - (Staff may go first- 7:30 ish)
9:30-10:30 am High School (staff may go first) Not sure on that time. It may change.
9:30 - 11:00 am Brock Intermediate (staff may go first)
1:00-2:00 pm Brock Jr. High Campus (staff may go first)
