Please Complete Online Re-Enrollment for 2021-22 School Year
Online re-enrollment for existing students. Please complete the online re-enrollment forms for enrolling your student(s) in the 2021-22 school year.
- Household Data: Make any changes to your address, parent/guardian information, etc. Please note: your personal email address cannot be changed during Online Registration. Contact your child's school if you need to change your personal email address.
- Student Information including Emergency Contacts. Please note: non-resident Parent Information changes must be made by the non-resident parent by calling the school).
- *Enrollment/Re-Enrollment Information Form: Make sure to answer each question “yes” or “no” and provide any documentation to your child's school as needed.
- *Health Information Form including allergy information, asthma history, and medical history information.
- *Transportation Form: Please select the appropriate before school and after school transportation for each day of the week.
- *Technology Usage Form (if applicable, for 2021-22 K-6th and 9th grade students).
- *For PCHS Students Only: Parent A+ Agreement (for 2021-22 9th grade students) and Student Activities Eligibility and Code of Conduct Forms.
*These forms are student-specific. If you have multiple children in the District, you will complete a form for each child. The child's name will be listed in the top left-hand corner.
Follow the steps below to complete the re-enrollment process ASAP. Click here for a step-by-step guide with screenshots.
- Log on to Parent Portal via a web browser. If you need assistance with Parent Portal, contact your child's school.
- Find and click on the Online Registration link within the yellow bar.
- Complete each form in the list by clicking "Edit." When each form is complete, check the check box (next to the red text) to authenticate the entries, and then click the "Next" icon to proceed to the next form.
- Click the "Save" button if unable to complete all the forms at one time.
- When all forms are completed, click "Return to List." Then click "Submit Forms" to submit your information.
- Upon submitting your online enrollment, you'll receive a confirmation on the screen, Click "Ok" to complete your submission.
- Finally, a message stating that your submission was accepted will display on the screen.

*Incoming Kindergarten students and new students to Platte County R-3 will complete school enrollment forms via digital pdf or hard copy forms during Kindergarten Kickoffs or during new student enrollment dates in August.
*Online Registration can only be completed by a resident parent.
*Parents can use their child's Chromebook to complete Online Registration.

Electronic Proof of Residency
As part of a continued focus on maintaining fiscal accountability, ensuring our tax dollars are spent on Platte County R-3 students, the District requires proof of residency for all returning students annually. Please see attached instructions and webpage. If you have multiple students in the District, only one proof of residency is required.
Current and new students must complete an Affidavit of In District Residency Form AND provide one of the following documents.
- Utility bill (electric, gas, sewer, or water). No final or disconnect notices accepted.
- Current residential lease agreement signed by resident/parent & landlord.
- Residential housing contract (purchase).
Document provided for verification must:
- Include parent(s)/guardian(s) name(s) and current address.
- Be dated April 1, 2021 or later, but also dated within 30 days of date of verification. For example, if the document is provided on July 25, 2021, it must be dated June 25, 2021 or later. As further clarification, a new student enrolling on November 1, 2021, must provide an acceptable residency verification document dated no earlier than October 1, 2021.
Electronic Proof of Residency: April 14 - July 16, 2021 - Proof of Residency accepted electronically via email to ONE of your students' schools. (returning students only)
