The Problem Solving Protocol Form replaces the Intervention Team Request Form and the two observation forms.

MTSS Rationale
The MTSS process should be used thoughtfully. It is a response to intervention plan that should only be used when solid tier 1 instruction and supplements to the core fail to close learning gaps that are larger than a two year increment.
To determine if you are providing solid tier 1 core instruction, then ask yourself the following questions:
Are 80% of my students proficient on PM Assessments, Common Formative Assessments, Common Summative Assessments?
Have you intensified core Tier 1 instruction strength and dosage, considered alignment, support and transfer learned skills, improve comprehensiveness, enhance behavioral and academic support?
Based on data, what areas of core instruction need to be intensified?
*If you answered no to these questions, then please check your core and practice the 80/20 rule.
Tier 2/Tier 3 Placement Test
MTSS Must Do's
1. Complete the Problem Solving Protocol plan for each MTSS student.
2. Create a plan in Synergy/or Close out the plan if MTSS is no longer necessary(pending parent approval)
3.MTSS data should be updated in Synergy weekly.
4.Parents should be updated on MTSS plans monthly..
5. Hard copy data should be collected in the event that Synergy does not work, or loses the information...ect!
6.Tier 2: 3-5 times a week, 10-30 minutes, small group 5 or less: 1 data point per week, at least 4 weeks
7.Tier 3: 4-5 times a week, 20-60, small group 3 or less: 1 data point per week, 15 weeks - 30 weeks
8. If a student is on MTSS currently and there is not a plan available in Synergy, then please create a plan and put it in Synergy!
Updating MTSS Plans
Each MTSS plan needs to be updated. Please complete the Problem Solving Protocol to update each MTSS plan. Once this has been completed, please place the original in my box.

Please follow the directions below to create an MTSS plan in Synergy
Closing out an MTSS plan
* Closing out MTSS plans must be based on the most up to date data points.
If it is determined, based on current data, that the plan should be closed out, ensure that the teacher enters the current data into Synergy, schedule a meeting to share results with the parent and student using the Parent Notification of MTSS Meetings. If this is a student with an IEP, request that the ESE secretary schedule an Amendment IEP meeting to share the results with the parent and student. At the meeting with the parent, use the Analysis of Academic Interventions and Recommendations or Analysis of Behavioral Interventions and Recommendations form to document the close-out of the plan. Ensure that the teacher closes out the plan on Synergy, pending parent approval.
For plans that are continuing...
Schedule a meeting with the teacher, MTSS Lead Administrator, and Intervention Team Facilitator to complete the Problem Solving Protocol for Individual Student Concerns: Academic or Problem Solving Protocol for Individual Student Concerns: Behavior for plans that are continuing. These will be completed for all MTSS plans, including parent request for evaluation.
Consult the Reading Decision Tree or Math Decision Tree to determine if the intervention implemented in 2021-22 is on the 2022-23 Reading or Math Decision Tree and is still appropriate for the student based upon current data.
If yes, ensure that the teacher updates the plan in Synergy and continues.
If not, carefully review the intervention options on the CCDS Reading Decision Trees or the CCDS Math Decision Trees to determine the most appropriate intervention and ensure that the teacher updates Synergy with a new plan and continues.
