the three mile island
the three mile island was a nuclear energy facility that melted down because of the reactor's core overheated. The valve for the coolant switched open and the coolant started to leak out. Only problem, the control center didn't know that it was happening at the time. Alarms started to go off and the reactor exploded.

This is the aftermath of the explosion of the building. The explosion ripped a hole in the side of the reactor building.
