Monday: Time to Send Folders and Reading Logs
IMPORTANT FORMS REGARDING THE FIELD TRIP
In your child's Monday folder, you will find the following forms:
The THREE forms must be returned to class by TUESDAY, APRIL 2ND.
- PERMISSION SLIP FOR EXCURSION - Your child's name goes in the first blank. Money - $11.00 (exact change only - I do not have any way to make change). At least one emergency phone number. Dated and Signed
- LUNCH ORDER - Even if your child is bringing lunch from home, please check the box that indicates that option. I have to know what each student is doing; please carefully review your options. I can promise you that you will not have time to order something from their little lunch spot; it takes too long, and we only have a thirty minute window to eat.
- CHAPERONES - Because I work at the museum, our CEO is graciously allowing our chaperones to come in with us at no charge. However, this does not mean that you can bring the whole extended family. Siblings do not qualify as a chaperone. If you have a question or need clarification, please let me know. Please return this form circling YES or NO and completing the requested lines. Again, I have to have an exact number of who is going. We have to know our numbers prior to going on the trip.
- Field Trip Chaperone Information is yours to keep. It is a separate form; originally I put it on the back of the chaperone form not considering that that form must be returned, so you would not be able to keep the needed information. Whoops!
TUESDAY, APRIL 16TH - FIELD TRIP - SCIENCE MUSEUM OKLAHOMA
We are going to visit my science museum. I've worked there in some capacity since 1996. 🙂
Time: leave school at 9:15 SHARP! - leave museum at 1:30
Student Cost: $11.00 (Museum Entry 8.00 Transportation 3.00)

*Chaperones MAY NOT ride the bus. Chaperones will be in charge of a small group of children.
LUNCH: Pack your own lunch or order one from our cafeteria. If you order from the cafeteria, they will simply charge your child's lunch account.
You will receive a registration packet in April! I am so excited. I HOPE YOU CAN JOIN US FOR THIS WONDERFUL TRIP! (I think it's the best, but I might be a little biased.)
