The Forms
The following forms are required when filing for bankruptcy:
Petition – the overview document for the whole bankruptcy filing. It lists, among other things, the names of those filing, the type of filing, partial social security numbers, an address, county of residence, the bankruptcy chapter, nature of debts, approximate number of creditors, and exhibits, including proof you have completed a credit counseling class.
Schedules – includes an overview and more in-depth listing of your assets and liabilities. For example, you need to list the real property you own, personal property, and property you claim as exempt. You also need to list creditors you owe money to and creditors who hold claims against your property (such as mortgage and car loan creditors).
Statement of Affairs – provides an overview of your complete financial history for certain periods before filing for bankruptcy. Specific information that needs to be given here is information on income gained from employment for the designated time period, other income, payments made to creditors, suits and garnishments, repossessions and foreclosures, gifts and charitable contributions, losses, and payments made related to debt counseling and bankruptcy, among other things. Statement of Intention – deals with your intention as to debt secured by property such as car loans and mortgages. Statement of Social Security Numbers – a form listing all of the social security numbers of those requesting the bankruptcy protection.

Declaration of Evidence of Employers Payments within 60 days – a statement along with accompanying evidence of all payment advices, pay stubs or other evidence of payments received by you from any employer within 60 days prior to the filing of the bankruptcy.
