Hemorrhoids split mainly in two typologies, internal and external
Internal hemorrhoids develop inside the ano-rectal canal (that is to say after the rectum, in the last three centimeters of the alimentary duct), they are not visible to the naked eye and are very often asymptomatic
External hemorrhoids are evident outside of the anus with the prolapse. They are covered by sensitive dermis and show as hard bulges and sometimes aching
Internal and external hemorrhoids are classified on a scale of four stages:
I stage- At this stage hemorrhoids are internal, they can bleed during bowel movements, but you wouldn't feel any pain
II stage-At this stage hemorrhoids are still internal but tend to prolapse (pour out from the anus) during defecation to get back inside spontaneously
III stage- Hemorrhoids prolapse spontaneously during defecation and need to be gently repositioned inside the anus with your fingers
IV stage- Hemorrhoids can't be repositioned inside the anus and firmly stay outside. At this stage pain can be even sharp
Going back to internal hemorrhoids, it is to be said that being under the mucosa inside the anal canal, they can be rarely recognized by a simple digital inspection, unless there are thrombosis.
However, when these veins dilate they have been inside the rectum for long, they can increase in volume and start to protrude from the anus, until they become visible and recognizable. Indeed, the surface of prolapsed internal hemorrhoids is covered by mucosa, while external hemorrhoids are covered by skin. When these medicines are used on the skin of external hemorrhoids, only a small quantity get absorbed and poured in the blood.
