Student's COVID-19 Vaccination Card

As we continue to keep you updated and adhere to the COVID-19 guidance provided by the California Department of Public Health (CDPH) and Los Angeles County Department of Public Health (LACDPH), we have created a procedure that will improve the process of identifying and notifying families in the event of a close-contact exposure to an individual with COVID-19 at our school sites.
Parents/guardians can now upload their student's COVID-19 Vaccination Record Card through the Aeries Parent Portal - https://bit.ly/aeries-portal

Parents/guardians can submit their student’s COVID-19 Vaccination Record Card to the MVSD Family Center, 2740 Mountain View Rd, El Monte, CA 91732 from 8:30 am to 4 pm, where staff will make a photocopy of the card.
As you may know, the current guidance for fully vaccinated students who are identified as close contacts to a positive COVID case is different from the guidance for students who are unvaccinated. Fully vaccinated students who are identified as close contacts may continue to attend school and participate in extracurricular activities as long as they are asymptomatic (no symptoms).
We are encouraging parents/guardians of fully vaccinated students to provide the school with verification of their student being fully vaccinated through the Aeries Student Information System. Having this information in advance will provide important data for the school administrators and health office staff who are conducting contact tracing and notifying parents and guardians about quarantine guidance.
While providing your student’s COVID-19 Vaccination Record Card is voluntary, if your child is fully vaccinated, we request that you follow the directions provided as an attachment (and available on the MVSD website) to provide the school with verification of your child's status as fully vaccinated. Please be assured that this personal and confidential information will only be accessed in the event your child is identified as close contact to an individual who has tested positive for COVID, and only authorized staff who are conducting contract tracing will have access to this information.
Note: Individuals are considered fully vaccinated 14-days after their 2nd dose of a 2-dose series and Booster for 12 and up if eligible.

If you lost your CDC vaccination card, you may be able to obtain a copy of your vaccination record using California's Digital COVID-19 Records at https://myvaccinerecord.cdph.ca.gov. You can upload your record from that system to the Aeries Parent Portal.
