Physical Forms
All students are required to complete concussion training prior to trying out for sports. Students entering 9th grade and new students to Prince William County Schools are required to attend a face-to-face concussion training offered by a Prince William County Schools' Certified Athletic Trainer. If 10th-12th grade students have previously attended the high school face-to-face concussion training, they may complete the online concussion training.

Colgan Clinic
Welcome back to the 2019/2020 school year. Ginger Sorensen (clinic aide) and Pam Johnson (School Nurse) are so excited about this upcoming school year. Our main function in the clinic is to take care of health related problems and needs that occur during normal school hours. Students and parents are asked to contact the school nurse if they have any questions concerning the student's medical needs. During the school year, health screening is provided for selected students. The screening includes vision and hearing. When conditions are found that merit attention, parents are notified. For health and safety reasons, ALL STUDENTS MUST HAVE A PASS TO THE NURSE. It cannot be a stop off between classes.
MEDICATIONS - State Board of Education policy requires a note for the administration of medication by the school nurse. Medications prescribed by a doctor must be brought to school in the original container with proper labeling along with a completed medication administration form. In addition, controlled medications must be brought in by an adult and handed to the nurse with the exact count of pills in the container. Students must have a medication authorization form filled out by the parent/guardian for Tylenol, Ibuprofen, or Tums to be given during the day. Students may carry an asthmatic "quick relief" inhaler or an emergency epi-pen if:
* There is a signed Asthma Action Plan or Allergy Plan on file
* They have a medical need
* The medication has the prescription label
* The school nurse is aware they have the emergency medication
Students who use the epi-pen should always report to the school nurse immediately or if a student uses the "quick relief inhaler" and they do not experience relief. All other medications are to be kept in the nurse’s office. STUDENTS ARE NOT PERMITTED TO CARRY ANY MEDICATIONS, including Tylenol, Advil or Midol to take on their own. All medications are to be administered by the school nurse.
EMERGENCY CARD INFO - An emergency card must be completed. We must have pertinent information to provide optimal care and protection for our students. PLEASE RETURN THEM AS SOON AS POSSIBLE. If parents / guardians change their phone number, address, or emergency numbers it is critical that the nurse know immediately in order to ensure the students safety.

Free and Reduced Price Meals
Free and Reduced Price Meal applications were sent via email or US mail by the School Division to families with children registered in Prince William Count Public Schools. If you did not receive an application, you may complete the application electronically. Visit the School Division website at www.pwcs.nutrition.com to access the electronic application.

Family Life Education Program notification to parents
Family Life Education Program notification to parents
Principals: Per the PWCS “Family Life Education (FLE) Guidelines,” schools should notify parents about FLE opt-out procedures through a September edition of their school newsletter and the school website.
The Parent Letter/Opt-Out form is part of the PWCS student “Code of Behavior & Forms,” located on the Code of Behavior & Forms page and on the Science and Family Life Education page (choose the Family Life Education tab). Objectives by grade level are also located on this page. Parents should visit the site and review the objectives for their child’s grade level or course. Parents may exercise opt-out for any FLE objective by returning the opt-out form to the appropriate teacher at any time during the school year, but preferably within the first two months of school.
It is strongly recommended that principals communicate to parents their school’s timeline for Family Life Education and provide notification to parents again shortly before actual instruction. Parents should have the opportunity to review curriculum and instructional materials. Schools may choose to schedule video preview events. A complete copy of all printed materials and a description of all audio-visual materials should be kept in the school library or office and made available for review during school hours before and during the school year.
