Message From Nurse Cruz

Dear Parent/Guardian of 6th grade students,

Texas state law TAC 25 Rule §97.63 mandates students entering the 7th grade be completely vaccinated against communicable disease including tetanus and meningitis. The additional Tdap and Meningitis vaccines must be obtained prior to entrance into the 7th grade. Unvaccinated students will NOT receive a schedule and will not be permitted to attend class until evidence of vaccination is provided to the school nurse. These absences are not considered excused absences. Please send a copy of the updated vaccinations to the school nurse.

If you have any questions, regarding the required vaccines, please feel free to contact the campus clinic.

Thank you,

Nurse Cruz.

Lomax is offering a campus physical on April 12th with Dr. Kirkwood for the cost of $20 cash. Those wishing to do this will bring their completed forms, and money to their Coach prior to April 12th. Feel to email or call with any questions.
