Starting a New School Year
We understand that students may have some anxiety and worries about starting a new school year. If you feel that it would benefit us to know specific information regarding your child that you have not recently shared, please do not hesitate to reach out to our nurse, Beth Jones (bjones@sd13.org), or our Social Worker, Sheila Zagotta (szagotta@sd13.org).
Once you receive your child's schedule, you may also share information with any classroom teacher if you feel it would benefit your child's experience at Westfield. We are all here to help! Please contact our Main Office at 630-529-6211 if you have any questions.
Parent Advisory Committee
Westfield is starting a Parent Advisory Committee this fall. This group of parents will work directly with Mr. Larsson to act as a communication link between the school and the community, help to develop solutions to address concerns of students, parents and community members, assist in offering advice on school issues, and promote parent and community involvement at Westfield. We are looking for four parents at each grade level to be part of this advisory committee. We will plan to meet every other month, starting in September. If you are interested in joining please fill out the Google Form by Friday, August 19.
Have a wonderful weekend!

FROM THE WESTFIELD NURSE
To All Incoming 6th-Grade Parents:
The Illinois School Code requires that all students entering 6th Grade have the following upon entrance to school:
- A current physical exam must be recorded on the approved form signed by your physician with a completed and signed parent section PHYSICAL EXAM FORM
- A complete record of mandated immunizations. In addition, a Tdap and an MCV4 vaccine are mandated for 6th grade
- A current 6th grader's mandated physical examination will also allow the student to participate in sports; another sports exam is not necessary
- A current oral health exam recorded on the approved form and signed by your dentist
- Mandated forms are included as part of the registration process; medical offices also have all the mandated forms
- Please, be aware that physicians’ and dentists’ offices are very busy during the summer: be certain to book your appointment early
About Your Child's Medication:
- - The district's medication administration policy mandates that all medication to be administered at school requires a MD order. This includes over the counter medications, and students may not keep OTC meds in their backpacks or in their lockers. This is an important safety concern! Please review the policy included in the registration process. The required forms are here and available when you register your student.
- - If your incoming 6th grader requires an inhaler, Epi-Pen, or any oral medication, please obtain the medication form and appropriate health care plan from your physician when your child has their exam.
- - Please note that all PE excuses need to be sent to the Health Office, not the PE teacher. A parent may excuse a student from PE for 3 days.

How Parents Can Log in to Otus:
- Sign in to Otus (if you have not set up your family account, please follow the directions in this video or this article to do so.)
- PLEASE READ FIRST: Your student already has an Otus account from the school district. To avoid confusion, it's best to use your own name rather than your student's name when creating the Family account. Both the video and the article have the same information. Please view whichever is more convenient for you.
- (If you have created your family account, but need to connect your student's code, have your child log in to Otus and click Edit Profile. Their student code is located at the bottom of that page.)
- To check your child's progress on Otus, click on Gradebook, then choose which class you would like to see progress in.
- To check your child's completed assignments on Otus, click on Gradebook, then click the three dots in the top right corner to change the "Grading Scale" to "Completion Scale."
