Oregon Healthy Teen (OHT) Survey
On February 12, our school will be administering the Oregon Healthy Teen (OHT) Survey. The survey data are used and applied at the community, county, regional, and state levels to assess youth needs, develop and evaluate the effectiveness of a variety of projects and programs that promote healthy adolescence in Oregon prevention, mental health and suicide prevention. After completion of the survey, our school will receive a data-rich profile report of findings, which will feature a new school climate and safety/bullying section, among other great data.
The survey is scheduled to take place during advisory. We are asking for your help in administering this important survey.
Approximately three days before the survey takes place, you will receive a packet of survey materials. The packet will include instructions for administering the survey via _<paper/online surveying>_, and the necessary materials to do so. In preparation for the survey, there are a few things that you should know:
- Passive Parental Consent. Passive parental consent forms will be distributed to parents. You will be notified if any students in your class do not have parental consent to take the survey.
- Scheduling. The students will need an entire class period to complete the survey.
- Alternate activity. The student survey is voluntary. An alternate activity must be provided for the students who elect not to participate. The alternate activity should be something the student can do quietly at their desk (i.e., reading or written activity). Please be prepared to assign an alternate activity for your class on the day the survey is given.
I encourage you to contact me at with any questions you may have. Thank you in advance for your valuable assistance on this research project. We couldn’t do it without you!
Sincerely,
Cindy Schubert
