Report Cards will be distributed to students on Tuesday June 26th. If your son/daughter does not pick up their report card, it can be picked up in the main office. Grades can also be viewed on the parent portal.

2018-19 student schedules will be available on Monday, June 25th. They will be distributed at the start of the school day. If you have any questions in regards to your child's schedule please contact their school counselor.

Mandated Screening
Screenings are conducted in Tyngsborough Public Schools annually in accordance with the Massachusetts Department of Public Health Regulations (refer to guidelines below). Screening tests are not diagnostic in nature. They are designed simply to indicate students who may need further attention in a specific area.
If a parent/guardian does not wish to have this screening done at school they must send:
1. a written request to exclude their child from participating in school screenings AND
2. written documentation from the family physician documenting their results of the screening.
Screenings are performed at THS throughout the school year as the student's schedule allows.
Postural – students will be screened in grade 9 for scoliosis. Complete privacy is afforded the students during this examination. If your child is currently under the care of a physician for scoliosis please submit documentation for their health record.
Vision and Hearing – will be screened during grade 10.
Growth and Development – Height, Weight, Body Mass Index (BMI) and corresponding percentile will be calculated for students in grade 10.
Parents/legal guardians will be notified of any findings that necessitate medical follow-up.
Screening, Brief Intervention, and Referral to Treatment (SBIRT) – will be performed for all grade 9 students. The SBIRT protocol facilitates a one-on-one conversation between each student and a school nurse or counselor in an effort to prevent or at least delay the start of using harmful substances, such as alcohol and marijuana. SBIRT does not result in disciplinary action. This program focuses on reinforcing healthy decisions and addressing concerning behaviors around substance use with the goal of improving health, safety and success in school. We will use the CRAFFT II screening interview, which is the most commonly used substance-use screening tool for adolescents in Massachusetts. The screenings are brief (5-10 minutes) and will be conducted in a confidential setting in private one-on-one sessions with one of the nurses or counselors trained in SBIRT screening. Students who are not using substances will have their healthy choices reinforced. The screener will provide brief feedback to any student who reports using substances, or is determined to be at risk for future substance use. What your child tells the screener is confidential and will not be shared with any other person without prior written consent of the student, parent or guardian, except in cases of immediate medical emergency or when disclosure is otherwise required by state law. No written record of the results of this verbal screening is kept with information that identifies any individual student. Screening results will not be included in your child’s school record. You can view the CRAFFT tool and other SBIRT resources on the www.masbirt.org/schools website. As with any school based screening, you have the option to opt your child out of this process. Physical Examinations
A periodic physical exam is critically important for all children and adolescents. The objective is to identify and follow up on health conditions that may adversely affect students' well-being and ability to learn. While parents/guardians have primary responsibility for their children's health, the school is responsible for the care, safety, and well-being of students in the educational setting. Therefore, a student's health history and physical examination results are necessary components of the school health record. Under the guidelines of the Massachusetts Department of Public Health and the Massachusetts Department of Education, it is the policy of the Tyngsborough Public School District that students are required to submit a current Physical Examination report to have on file in the nurse's office upon entry into JUNIOR year. Examinations must be from within the previous twelve months to be considered current and must include a certificate of immunization. Documentation of Physical Examination is also required prior to a student's participation in competitive athletics, on an annual basis. Please do not submit Physicals to a coach. This documentation can be forwarded to the e-mail, fax, or mailing address listed below during the summer or to the School Nurse when your child returns to school. Early submission is very helpful in avoiding delay in your child participating in try-outs and starting the season.

Students who have been prescribed an EpiPen
A Food Allergy & Anaphylaxis Emergency Care Plan must be fully completed for the start of each new school year including Parent/Guardian Signature and Physician/Healthcare Provider Signature. If your child’s physician has his/her own Allergy Action Plan it will be accepted as long as it is signed by physician AND parent/guardian. Please submit the first day your child returns in August with an unexpired EpiPen. I also encourage students prescribed an Epi Pen carry one at all times.
Students who have been prescribed a rescue inhaler
A Student Asthma Action Card must be fully completed for the start of each new school year including Parent/Guardian Signature and Physician/Healthcare Provider Signature. If your child’s physician has his/her own Asthma Action Plan it will be accepted as long as it is signed by physician AND parent/guardian. Please submit the first day your child returns in August. It is acceptable for your child to carry the inhaler as long as this is indicated on the plan. However, if your child has difficulty remembering to keep inhaler with them at all times I encourage you to submit one to be stored in the Health Office. This form must also be on file if your child uses an inhaler while participating in extracurricular athletics.
Students diagnosed with a Seizure Disorder
A Seizure Action Plan must be fully completed for each new school year including Parent/Guardian Signature and Physician/Healthcare Provider Signature. If your child’s physician has his/her own Seizure Action Plan it will be accepted as long as it is signed by physician AND parent/guardian. Please submit the first day your child returns in August with any emergency medication indicated on the plan.
Students requiring medication during school
School policy prohibits students from carrying medication other than emergency medication such as an EpiPen or rescue inhaler (which as stated, require the above documentation be on file with nurse). Our school physician provides us with a standing order for ONE DOSE of the following medications during a school day as long as the parent documents consent on the Student Information Form on the Parent Portal:
1) Acetaminophen according to manufacturer instructions (MAX dose 650mg) OR
2) Ibuprofen according to manufacturer instructions (MAX dose 400mg)
3) TUMS or similar antacid according to manufacturer instructions dosage for weight/age
4) Benadryl (Diphenhydramine) according to manufacturer instructions dosage for weight/age
5) Caladryl Clear or generic derivative according to manufacturer directions for relief of itching from minor skin irritations
6) Topical Antibiotic Ointment may be applied according to manufacturer directions for minor cuts, scrapes, or burns to help prevent infection.
If your child requires more than one dose of the aforementioned medications during a school day, or a dose that exceeds the above listed dose, a Student Medication Administration Order must be completed by their physician and signed by parent/guardian.
All other medication, prescription or over-the-counter (OTC), requires the parent submit the medication in a pharmacy labeled container or the original manufacturer’s container with a Student Medication Administration Order completed and signed by their physician and parent/guardian. No more than a 30 day supply should be submitted and it must be submitted in person by parent/guardian with a signed note documenting amount submitted.
