2024-2025 Registration For All New Students
Requirements for a successful registration experience!
STEP 1

Please bring with you the students birth certificate and a PAPER copy of the students current immunization records from the doctors office. Here are the requirements for entering Kindergarten:
- 4 DTAP (Diphtheria, Tetanus, and Pertussis) – 4th must be on or after the 4th birthday.
- 4 Polio – 4th must be on or after the 4th birthday.
- 3 Hepatitis B
- 2 MMR (Measles, Mumps, Rubella)
- 2 Varicella
ALL STUDENTS are required to complete the Health History Form every school year. This is a separate form from supplying the students current immunizations or physical, of which you get from the doctors office. The health history form is completed by the parent or guardian.
All students are required to complete the Release of Records Form. (educational and medical purposes)
ALL SINGLE PARENTS without custody papers/parents whose name does not appear on the birth certificate at the time of registration, are required to complete the Parent/Guardian Custody Form. *Notarization is required.
The Parent Input Packet and Dental Form are for students entering KINDERGARTEN. Please print them out and bring the Parent Input Packet to our Registration Appointment.
The Dental form and a copy of your child's most recent physical are to be submitted the 1st week of school.
STEP 3
RESIDENCY REQUIREMENTS
Homeowners are required to bring in their current mortgage statement or current tax bill, current utility bill, an additional piece of mail and a valid ID with your current address. (This excludes current pre k families)
Renters are required to provide their current lease and renewal letter if not current, current utility bill, additional piece of mail and a valid ID with your current address.
The lease verification form is completed by ALL renters. Notarization of the lease verification form is only required for those presenting a month to month lease. It is a legal document, notarized by you, not the landlord.
Only students whose families are living in a Multiple Occupancy situation (living with a resident/taxpayer/homeowner) need to print out and have notarized, the *Multiple Occupancy Packet. The required documents are listed on the form. Please note that the tax payer must be with you at the time of enrollment.
If you do not have a printer, please stop by and pick up the forms or plan to complete them at our meeting. Some of the forms require notarizations, so please look over the forms and determine which pertain to your family/living situation.*
EVERYTHING NEEDS TO BE SUBMITTED AT THE TIME OF REGISTRATION AT YOUR SCHEDULED APPOINTMENT. (Required forms, birth certificate, paper copy of current immunizations, residency documents. If you are missing any of the requirements, you will need to have another appointment, so be prepared for our meeting.
The last step will be for you to complete an online enrollment form that we will send you the link, once your residency and immunizations have cleared. Once completed your student is enrolled, teachers selected and you will be scheduled for orientation.
