Pandemic Electronic Benefit Transfer (P-EBT)
Medication
Medication must have a written request signed by the parent/guardian and physician prescribing the medication. This request must identify the medicine, amount and time(s) to be given, and the length of time the student will be taking the medication.
-All medication needed to be taken at school should be in its original container & properly labeled.
-Students are not allowed to carry ANY medication (prescribed or non-prescription) to classes.
-Any exceptions must be approved by the nurse and principal with written request from the parent/guardian or student's doctor. The school nurse may make an exception on a temporary basis when unusual circumstances exist. Please see important documents.
- A physician’s authorization is needed for the following:
- Any medication given for more than 10 school days.
- Any controlled substance
- Emergency Medications- EPI PENS/ KITS or inhalers (all requiring doctors instructions)
- Annual updates for long term medications
Immunization
- The State of Texas mandates that students' immunization records be up to date and students will be excluded from school if they do not get the required shots.
- Immunizations are required to attend school. Please keep this in mind this school year as you visit the doctor and have the opportunity to receive these immunizations. Please send an updated immunization documentation as soon as possible. (Remember to keep a copy for yourself.)
- Notices will be sent home to all students who haven't received these required shots.
Too Sick For School?
For the protection of all students, a child will not be permitted to remain in school, and the parent/guardian will be notified, in the event of:
§ Fever or 100.4 or more
§ Suspected contagious illness
§ Vomiting
§ Diarrhea
§ Undetermined rash
§ Lice and/or Nits
§ Eye irritation not related to allergies
-If your child has any of the above symptoms before coming to school, please keep him/her at home. Your child must be free of fever for 24 hours prior to returning to school. When contacted by the school to pick up your child, it is important that you pick up him/her as soon as possible.
- Any release from school must have the verbal consent of the parent/guardian.
