5 Essentials Survey

Parents across Illinois will have an opportunity to participate in the eighth annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.

On behalf of the Illinois State Board of Education, the Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago. The survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains. These five indicators that affect and predict school success are:

· Effective Leaders

· Collaborative Teachers

· Involved Families

· Supportive Environments

· Ambitious Instruction

Prior research in more than 400 schools has shown that schools that were strong on at least three of these “5Essential” indicators were 10 times more likely to improve student learning gains in math and reading than those that were weak on three or more Essentials.

All teachers and 4th through 12th grade students will be responding to this survey. 5Essentials Reports will be generated for schools if their teachers and/or students meet the response rate threshold of 50 percent. These reports will be sent to schools and districts in April 2020 and will also be included in the State School Report Card.

Additionally, Millburn School District has chosen to provide the Parent Supplement survey. If at least 20 percent of Millburn Elementary parents complete this survey, a parent supplement report will also be generated. The parent data will not be reported on the State Report Card.

Your participation in the parent portion of the survey will help us understand the conditions at our school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.

*****The Parent Survey Supplement is now live through February 14, 2020. To take the survey please visit or click the button below and select the appropriate survey to begin.

Read to Succeed Program

The 2020 Six Flags Read to Succeed program has begun! NEW this year are eReading logs, which are being sent home with all students this week. All directions are on the form, along with a unique code for your child's teacher. (All teacher codes are different.) Parents will need to register their child/children on the website and enter their child's teacher's unique code. Registration can be done on either a computer or smartphone. Simply keep track of your child's reading and record it onto the website by April 17, 2020. Students who read 6 hours will receive a FREE ticket to Great America. If students choose to read more than 6 hours, they can earn discounts on additional tickets for family members, season passes, and memberships. What a great way to earn a reward for all the hard work and reading that is being done by our MES students! If you have any questions, feel free to contact your child's teacher or Renee Marasco at

Feel free to use your read steps towards this!

Student Council Collection

Student council would like to thank you for all your candy donations! We were able to fill about 25 boxes with candy! All of the candy was donated to Operation Gratitude and Soldier's Angels.

We are currently collecting for the following:

Orphans of the Storm Shelter: We are collecting blankets, dog and cat treats, as well as dog and cat toys to donate to the Orphans of the Storm Shelter. We will be collecting from December 9th to December 13th. Blankets can be new or used (in good condition), the shelter requests nothing too large or heavy.

Toiletries: If you are traveling this holiday season, please bring us your hotel toiletries. Student council will be collecting them after Winter Break to give to those in need.

Dismissal Changes

Any changes to a child's dismissal procedures need to be communicated to the office by noon. Once we receive this communication, our office staff will contact the teacher to inform them of the change. We have encountered parents emailing teachers during the day about changes, but emails may not be checked until the end of the day after students have left. Contacting the office assures that the change can and will be done.

Please direct any changes to Mrs. Hayes:

If you are changing any of your plans with MBAC, it is best to contact Mr. Slaby directly, in addition, to contacting the main office.

Drop Off/ Pick up Procedures

We are seeing more and more families drop off students in the morning. Please remember to be patient when waiting to drop students off. To avoid students walking between cars, do not double park in the drop off lane. If you do park in one of our parking stalls, use the crosswalk to escort your child to the sidewalk safely.

When picking up your child, all cars MUST park in our parking lot and walk in to sign their child out. Parking a vehicle along our fire lane is prohibited. Thank you for your cooperation.

Big picture

Kindergarten Lunch Helpers/ Winter Weather

Our Kindergarten students have done a great job with learning the lunchroom procedures. We are seeing students not need as much help, becoming more independent with getting the lunches and opening their cold lunch items. Yet, as we approach winter, our next obstacle is snow gear. When students leave the cafeteria, they proceed to get on their winter coats, gloves, hat, and snow pants on before going out for recess.

We would appreciate any parent helpers to come on cold weather days around 11:15 to help our Kindergarteners get their gear on. It would only be a few moments of your time, but the more hands the better!
