5 Essentials Survey
Parents across Illinois will have an opportunity to participate in the eighth annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.
On behalf of the Illinois State Board of Education, the Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago. The survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains. These five indicators that affect and predict school success are:
· Effective Leaders
· Collaborative Teachers
· Involved Families
· Supportive Environments
· Ambitious Instruction
Prior research in more than 400 schools has shown that schools that were strong on at least three of these “5Essential” indicators were 10 times more likely to improve student learning gains in math and reading than those that were weak on three or more Essentials.
All teachers and 4th through 12th grade students will be responding to this survey. 5Essentials Reports will be generated for schools if their teachers and/or students meet the response rate threshold of 50 percent. These reports will be sent to schools and districts in April 2020 and will also be included in the State School Report Card.
Additionally, Millburn School District has chosen to provide the Parent Supplement survey. If at least 20 percent of Millburn Elementary parents complete this survey, a parent supplement report will also be generated. The parent data will not be reported on the State Report Card.
Your participation in the parent portion of the survey will help us understand the conditions at our school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
*****The Parent Survey Supplement is now live through February 14, 2020. To take the survey please visit https://survey.5-essentials.org/Illinois/ or click the button below and select the appropriate survey to begin.
For more information about the Illinois 5Essentials Survey, or to view previous years’ Reports, please visit https://www.5-essentials.org/illinois. If you have any questions or concerns, please contact 5Essentials Client Services at 1-866-440-1874 or 5essentials@uchicago.edu.
Read to Succeed Program
Feel free to use your read steps towards this!
Student Council Collection
Student council would like to thank you for all your candy donations! We were able to fill about 25 boxes with candy! All of the candy was donated to Operation Gratitude and Soldier's Angels.
We are currently collecting for the following:
Orphans of the Storm Shelter: We are collecting blankets, dog and cat treats, as well as dog and cat toys to donate to the Orphans of the Storm Shelter. We will be collecting from December 9th to December 13th. Blankets can be new or used (in good condition), the shelter requests nothing too large or heavy.
Toiletries: If you are traveling this holiday season, please bring us your hotel toiletries. Student council will be collecting them after Winter Break to give to those in need.
Dismissal Changes

Drop Off/ Pick up Procedures
We are seeing more and more families drop off students in the morning. Please remember to be patient when waiting to drop students off. To avoid students walking between cars, do not double park in the drop off lane. If you do park in one of our parking stalls, use the crosswalk to escort your child to the sidewalk safely.
When picking up your child, all cars MUST park in our parking lot and walk in to sign their child out. Parking a vehicle along our fire lane is prohibited. Thank you for your cooperation.
Kindergarten Lunch Helpers/ Winter Weather
We would appreciate any parent helpers to come on cold weather days around 11:15 to help our Kindergarteners get their gear on. It would only be a few moments of your time, but the more hands the better!
Millburn WebStore
Millburn WebStore/Outstanding Fees
The Millburn Webstore is available! Parents can now pay for activity fees, sports fees, and more! Simply go to store.millburn24.net to access the Millburn WebStore. In the Millburn Webstore, you will have the option to make payments with a credit card, bank account, or for some items, your child’s PushCoin Wallet. To add funds to your child’s PushCoin Wallet, go to www.pushcoin.com.
Please check your child’s PushCoin account weekly to view any fees your child may have incurred (e.g., lost library book, MBAC fees, lunch overages). If you have not created a PushCoin account for your child, please visit www.pushcoin.com, click "I don't have an account," and follow the prompts to set up your child's account. Once you’ve created a PushCoin account for your child(ren), you will receive monthly emails from PushCoin if your child has an outstanding fee balance. Note that low balance lunch reminders are no longer sent home with your child. However, you can set a low balance or negative balance notification through PushCoin. This will be sent to the email address you provide.
