Final Report Cards
Important Health Reminders
Attention ALL Parents:
IF your child will need medication for the upcoming 2021-2022 school year, you will need to complete the BASD medication authorization form and emergency action plan (if applicable). Forms can be found on the BASD website under Health Services from the websites main page.
Medication authorization forms must be renewed each school year. Please have your doctor complete the top of the form along with your signature. Your child will not be permitted to have medication at school without the proper authorization. Please return all forms directly to the MSW nurse’s office at the beginning of the 2021-2022 school year.
Immunization requirements for 7th grade entry
Attention Parent(s)/Guardian(s) of students entering 7th grade; your child is required to have 1 dose of tetanus, diphtheria, acelluar pertussis (Tdap) AND 1 dose of meningococcal conjugate vaccine (MCV) by the first day of school. At this time, the Pennsylvania’s Department of Health has not eliminated the state immunization requirements. This requirement applies even to students who are continuing with virtual instruction. This information can be found in 28 Pa. PA Mandated Dental and Physical Exams for 2021 - 2022 School Year
The PA School Code requires dental exams for all students entering Kindergarten, 3rd and 7th grades, and physical exams for students entering kindergarten, 6th and 11th grades. For students entering these grades in September 2021, physical or dental exams that are completed on or after September 1, 2020 are acceptable. If you need a form for an upcoming appointment, you can download one from the BASD website @ http://www.boyertownasd.org under Resources - Nurse – BASD Health Service link. Completed physical and dental forms can be submitted to the health room at the beginning of the 2021-22 school year. Because the health room is closed for the summer, if you submit a completed form over the summer please retain a copy for your records.

Library Books Due
All library books and textbooks were due in May. If your child still hasn’t returned their library books, please remind them to do so ASAP. Students can drop their books off at the main office during our summer hours.
Eighth graders with outstanding obligations will lose privileges at BASH until all obligations are fulfilled. These lost privileges include: open campus, open lunches, after school events, and dances. These obligations follow the students through high school, and if they are not taken care of by graduation the student will not walk in the graduation ceremony. Diplomas are also held until the item(s) is returned or a replacement fee is paid, so be sure to take care of all obligations now!

Emergency Broadband Benefit Program
The Emergency Broadband Benefit Program is a Federal Communications Commission (FCC) program that provides a temporary discount on monthly broadband bills for qualifying low-income households. If your household is eligible, you can receive:
- Up to a $50/month discount on your broadband service and associated equipment rentals
- Up to a $75/month discount if your household is on qualifying Tribal lands
- A one-time discount of up to $100 for a laptop, tablet, or desktop computer (with a co-payment of more than $10 but less than $50)
Only one monthly service discount and one device discount is allowed per household.
The FCC expects enrollment for the Emergency Broadband Benefit Program to open on May 12, 2021. It will end when the fund runs out of money, or six months after the Department of Health and Human Services declares an end to the COVID-19 health emergency, whichever is sooner. There will be a zoom informational meeting on Monday May 24th at 7:00 pm for girls going into 9th through 12th grade who are interested in trying out for High School Volleyball in the Fall of 2021. Topics to be covered include off season workouts, tryout schedule, season schedule and PIAA paperwork. All girls planning on trying out and parents/guardians should attend the meeting.

Six of our sports teams are offering camps this summer where you can work on your skills in a fun and safe environment.

As a district, we have begun a 3-year implementation plan focused on Social Emotional Learning. Social and emotional learning (SEL) is the process through which children and adults acquire and effectively apply the knowledge, attitudes, and skills necessary to understand and manage emotions, set and achieve positive goals, feel and show empathy for others, establish and maintain positive relationships, and make responsible decisions.
