Dental Forms
Look in your student's Friday folder for a green-colored consent form offering a health service to help stop tooth decay. Please mark yes or no and return the form to the health office.
Home kits will be sent home at a later date. This service does not replace a regular dental check-up. It is recommended to visit a dentist once a year.
All students may receive this service!!
