Physical Examinations
The Department of Health requires physical examinations for students in grades *Kindergarten or 1st grade, 6th grades. Please, submit forms via email to schoolnurse@communityacademy.org
Dental Examinations
The Department of Health requires dental examinations for students in grade *Kindergarten or 1st grade, 3rd grade, and 7th grade. Please, submit dental forms via email to schoolnurse@communityacademy.org
Immunizations
Immunizations records are required for students Kindergarten, 1st grade, 7th grade. If your student is exempted from vaccines, please send in a copy of the exemption form. Please submit immunization records to the health office via email to schoolnurse@communityacademy.org
Please have your health care provider complete the physical and dental examination forms and return to the health office. If you have any questions, please feel free to contact the Health Office at (215) 533-6700 extension 1136.
Important Information
Flexible Instructional Days (FIDs)
Important Information: It is not expected that CAP will have traditional snow days this year. Instead, if the school needs to close using a Flexible Instructional Day (FID) for snow or another safety-related reason, students will complete schoolwork at home. Absence Notes
It's the law; when your child is absent from school, please remember to send in a note. For convenience, you may also simply send an email to absent@communityacademy.org.
- Parents, please be sure to write your student's first and last name on their personal items, including sweaters, blazers, lunch bags, etc. CAP is not responsible for misplaced items.

TRANSPORTATION:
Parents for Students in Grades 1-6: The Enrollment period for the flat rate is now closed. No other applicants are being accepted. If you have enrolled, please remember you must complete the MONTHLY payment form at the end of EVERY month, in order to receive payment by clicking the link below. Also, please note that the first payment may take up to 60 days to process.
UNIFORMS:
Students MUST be in full uniform each day - this includes ties, sweater (1st-8th)/blazer(grades 7th-8th) and proper school shoes. Gym uniforms must be a CAP gym shirt, or college bound shirt , navy blue CAP sweat pants and ALL black sneakers - no exceptions.

Counseling Corner
Setting a new year's resolution? Make it a SMART goal!
S- specific: make sure you know what actions need to be taken to achieve a goal. Ask yourself who, what, why, where, and when?
M- measurable: make sure your goal is something that you can accurately track and measure
A- attainable: don't set yourself a goal that you aren't going to be able to reach
R- relevant: make sure this goal aligns with broader goals you might have
T- timely: make sure you set an appropriate time frame for the goal
Making a SMART goal is a great way to become one step closer to achieving your goals!
