Notes from the Nurses...
8th Grade Washington DC Trip: If your child will be taking a prescription or over the counter medication on the trip, please see attached form.
Athletes
A sports physical must be on file in the health office dated after 1/1/18 to participate in the sports program for grade 6, after 1/1/17 for grade 7 and after 1/1/16 for grade 8.
Immunizations and Sport Physicals are available at Convenient MD Urgent Care 8 Loudon Rd. Concord. Open 8am-8pm seven days @ week. No appointment needed.
Inhalers
May only be carried, or kept in the health office, by students that have documentation on file at school. If you have any questions please contact Nurse Shannon
Health Care Provider Visits: When your child is seen for illness, immunizations, physicals/injuries please request written documentation from that visit and present it to the health office.
Prescription Medications: If your child is going to be taking a prescription medication at school, please provide the Nurse with documentation from the doctor. Medication Authorization forms are attached below if needed.
Over the Counter Medications: If you would like your student to take an over the counter medication here at school, please send in a signed note and have the student bring medication to Health Office. Students should not be carrying medications on them.
Elevator Use: The elevator is for students who have documentation from their doctor indicating injury, and necessity of use.
Staying hydrated: It's encouraged that students drink lots of water. A clear water bottle may be in the classroom with students.
