Important News from the Health Office for 5th Grade Parents
Today your child will bring home an envelope with important health information. The envelope will include a letter stating the immunizations we have on file for your child, information on the health requirements for 6thgrade, a physical examination form and a dental examination form.
The school nurse needs your child’s health records, so that your child can begin 6th grade. Please returncompleted exams to your school nurse before June 1st or to the middle school after June 1st. The physical and immunization requirements MUST be completed and turned in by the first day of school. Please make your appointment now.
If you have any questions regarding the health requirements, please feel to contact the Health Office.
The Dental Clinic is Coming to Lowell
The Du Page County Health Department Dental Health Services Unit will be at Lowell School on Friday, March 20. Forms were sent home last week. Participation in the program is optional. The Dental Unit staff will only see students with a completed, signed form. Forms need to be returned to the health office no later than Monday, March 16.
- Students meeting the income requirements will be eligible for a screening, sealants, and dental cleaning.
- If a student is eligible for free or reduced lunch they are automatically eligible for a screening, sealants, and dental cleaning. The income requirement on the form needs to be completed.
- The State of Illinois requires that all children enrolled in Kindergarten, Second and Sixth Grades have an oral health (dental) examination performed by a licensed dentist. The dental clinic screening would fulfill the requirement for those students who have not already submitted proof of dental examination.
- NO TREATMENT CAN BE DONE WITHOUT A PARENT SIGNATURE
This is a valuable service to our students. Please be prompt in returning the dental forms with your children. If you have any questions, contact the Lowell Health Office at 630-682-2085.

News From the Health Office
As you start spring cleaning to weed out the clothes your child has outgrown, please consider donating your gently used clothes to the Health Office. We have an abundance of shirts, but are especially in need of pants. Thank-you for your generosity!
