Madison CLUSTER 2021-2022
TRANSITION PLANNING
Transition planning involves helping students of differing abilities and their families identify the student’s areas of strength and areas needing support in addition to interests and preferences as they think about goals for their life after high school. This process includes looking at the supports the student will need during and after completing school including employment, education, and independent living. It's never too early or too late to begin planning for transition.
