Chesterton Middle School Newsletter - May 31, 2022

CMS families,
I hope you had a wonderful weekend. We had a great last week of school! We were able to celebrate with the students and enjoy the end-of-the-year student picnic despite the spotty weather. Congratulations to our 8th graders who have graduated and are off to CHS! Also, congratulations to the CHS senior class of 2022 and those of you who are enjoying your child's graduation!
A few quick reminders...
Parents may access student report cards on the evening of Wednesday, June 1, in Skyward under the Portfolio tab.
The CMS Main Office is still open from 7:30 am-3:30 pm, Monday through Thursday, until June 14 if you have any business to take care of. The office will then be closed and will reopen on July 11, with the same days and hours.
Back to School Night for CMS is August 8, from 3:00-8:00 pm. There will be more details to come. I will be sending email messages to communicate with you here and there over the summer as things come up. However, the next Friday Focus newsletter will be shared with parents on July 15.

National Junior Honor Society
5/24/22
Chesterton Middle School hosted their annual National Junior Honor Society (NJHS) Induction Ceremony at the school recently.
Eighth grade current members completed at least 15 hour of community service and participated in CMS different group projects throughout the year. CMS projects include a toy drive that helped Duneland families & different organizations around Porter County. They also collected stuffed animals to donate to Porter County's Reading Buddies program.

LIBRARY NEWS
Our Spring Book Fair was a great success! Students purchased over $2800 in BOGO books and our sucker library fundraiser will allow us to continue our book giveaways and semester prizes for our CMS students. This year the library has given away over 200 books throughout the year and during our Fall and Spring Book Fairs. We love getting books in our students' hands every chance we can.
Thank you for all you do to keep your kids reading. Check out some of what has been going on in the CMS Media Center this year! Have a great summer and thank you for supporting your school libraries and their librarians!

2022 CMS All Football Season Information
Information for the 2022 CMS Football season is available on the CMS athletics page.
2021-2022 ALL ATHLETES MUST HAVE A CURRENT PHYSICAL ON FILE TO PARTICIPATE!
ATTENTION!!!
Middle School Physicals will need to be turned coaches or the athletic department, prior to open gym, tryouts, or practices in order to participate.
There are a total of 6 forms that need to be completed. The actual physical is completed by the Dr. and the rest of the forms are completed by the parent(s) and the student. Physicals will not be accepted if there are forms missing!
*Physical checklist*
- Physical Examination from the Dr.
- History Form (It is 2 pages)
- Consent and Release Certificate (Make sure insurance information is complete)
- Concussion and Sudden Cardiac Arrest Form
- Co-Curricular and Extracurricular Activities Code of Conduct
Physicals and Consent Forms on the most current IHSAA documents must be dated after April 1, 2021 to be eligible for participation.

NorthShore Clinic for DSC Students
Duneland School Corporation has partnered with NorthShore Health Services and has established a school-based health clinic that is now located in Chesterton Middle School.
This partnership provides access to healthcare for ALL Duneland School students.
Some of the services include:
- Strep Throat
- Ear Infections
- Immunizations
- Sports Physicals
- Treatment for pink eye
- Rashes
- Behavioral health services and referrals
Duneland parents interested in using the school-based clinic for their child will need to have a signed consent form on file. Consent forms are available from your Duneland school nurse or can be obtained at the NorthShore clinic where the visit is scheduled.
Consent paperwork needs to be completed prior to an office visit. The paperwork is attached and can be completed prior to the visit to save time. CMS students can be seen in the clinic (with parent permission) without a parent being present. If you choose this option, the consent paperwork needs to be on file beforehand.
