The State of Illinois has the following health requirements for students at the following grade levels prior to registration:
Physical Examination and Immunizations
All Kindergarten, Sixth Graders, and all children entering an Illinois school for the first time.
Click here to access required form.
Dental Examination
All Kindergarten, Second, and Sixth Grade students. Click here to access required form.
Eye Exam by a Licensed Optometrist or Ophthalmologist
All Kindergarten students. Click here to access required form.
Please plan to make your doctor appointments now to ensure your student has the required immunizations.
If your student requires medication during the school day your physician will need to complete a Medication Authorization form. A parent/guardian Medication Authorization form will also need to be completed. These forms need to be updated annually. Medication must be sent in a properly labeled prescription bottle.
Emergency Action Plans for asthma, seizures, allergies and diabetes will need to be updated every school year. Your physician will need to complete these forms in order for your student to receive medication at school. Students who self-carry an asthma inhaler or epi-pen will need to have a Medication Authorization form on file.

Reminders About Student Arrival & Dismissal
As the weather continues to challenge us with bitter cold, ice, and snow, I want to remind everyone of some important things that need to be followed to keep our kids safe.
ARRIVAL
If you drive your child to school, there are two areas that you are allowed to drop students off.
- The first is our Quick Drop. This is located on 78th street along the red brick pavers on the north side of our building. This area is for drop off only. Students are to get out of your vehicle independently. This is not the area where parents should be exiting their vehicle to assist their child(ren). When this happens, it causes major congestion and can create a dangerous situation for the kids.
- The second is for parents who wish to park and walk their child up to the school. If you choose to do this, please park your car on the west side of Thomas, north of 78th Street. This area is not designated for Quick Drop (please see above). The street becomes very crowded and when students get dropped off in the street, it is very dangerous.
- If you choose to park on Thomas Ave and walk your child to the front entrance, please do not block residential driveways.
Please DO NOT drop students off in our front parking lot, or in front of the school. We have buses that go through that area and having students walking through the parking lot is dangerous at that time.
DISMISSAL
I anticipate that temperatures will begin to increase soon enough. Each year when this happens, we tend to have ice that begins to slide off the rear, and sides of the building. We are very aware of this and will block off areas with cones in our rear parking lot as well as our playground when this begins to happen.
The rear parking lot poses more than just falling ice danger. My teachers on duty have informed me that parents are parking in undesignated places in the back parking lot when coming to pick up their children in the afternoon. One of these places is in front of the dumpster. This blocks cars and prevents them from being able to leave the parking lot safely. This also blocks the safe passage of walkers who are heading to their cars.
PLEASE NOTE: Kids are not allowed to enter the back parking lot unaccompanied by a parent at dismissal. If you choose to park in the back lot at dismissal, please exit your vehicle and meet your child on the playground to take them safely back to your car. If you would prefer to have your child walk to your car, please wait in the car rider pick up lane which extends down 78th Street (behind the bus lane) and around the curve onto Ferdinand Street.
Thank you for your cooperation with these procedures. By doing so, you are helping us keep the kids safe!
Reminder - No Birthday (Food) Treats
If your child has a birthday during the school year and you would like to recognize the special day, please think about sending non-edible items such as pencils, stickers, erasers, etc.

Don't Be Late!
8:10AM 1st Bell
8:15AM Tardy Bell
2:30PM School Dismissed
All students arriving late to school (after 8:15AM) must report to the office for a tardy slip. Every 5th tardy a note will be sent home. Continuous tardiness may constitute other disciplinary consequences. Our goal is for all students to arrive at school on time each day. When students are late, they miss instructional opportunities. Please keep in mind that students arriving after 9:15AM are no longer considered tardy, but will instead receive a half day absence.
Being late is detrimental to all students' learning, especially if it occurs on a consistent basis. Be sure to do all you can to prevent tardiness. Your child will thank you for it!
