5Essentials Survey
Share your thoughts to help improve our schools!
D140 students, teachers, and parents will be given the opportunity to have a voice in improving their schools by taking the annual statewide Illinois 5Essentials Survey from February 3 through April 2, 2021.

These surveys will serve as an opportunity for schools to reflect on progress since the last statewide implementation. School and District leaders use their 5Essentials data to better inform their school improvement plans and help identify key strengths and areas for growth. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
5Essentials Student Survey
Students will take the 5Essentials Survey during their asynchronous time. Hybrid students will be given the information they will need to complete the survey from one of their teachers. Remote students will receive an email with instructions on how to take the survey.

Reminder about PE & Enrichment Classes
Please remind your student(s) that they need to attend a minimum of twice a week and complete their assignments for their PE, Health, and Enrichment class and complete their daily assignments. Participation points will be given when they attend these classes. Teachers will be live teaching every period and post the grade level they will be live with by block on their Google Classroom page. The schedule varies day to day to accommodate all student schedules. Please note on the schedule what time period starts.
Please find the PE and Enrichment schedules below. If your students are confused, they should email their PE/Health or Enrichment teacher directly. Teachers are excited to see your students on their Zooms.
