Afternoon Transportation Changes

By now, you have shared with your teacher how your student(s) will be going home each day. If you ever need to change your child's afternoon transportation, either temporarily or permanently, please complete the online form located here (or by clicking the image below) and also on our website under the Quick Links section. Our office staff monitors all submissions and notifies teachers directly of any changes made. Changes must be made before 1pm.

Please do not send a handwritten note regarding a transportation change to school with your child. These are sometimes misplaced in their backpack or not given to the teacher by the student. Additionally, emails sent directly to the teacher during instruction time regarding transportation changes may not be seen prior to dismissal.

Any question, please contact khays44@forsyth.k12.ga.us

Early Check Outs

If you need to check your child out of school early for a doctor or dentist appointment, please come to the front office with your photo ID. Please remember that our latest check out time is 1:45. No check outs are allowed after 1:45pm due to dismissal procedures beginning at that time. Thank you for planning accordingly.

Mandated Reporter Video - Please View Now

Do you plan on visiting or volunteering at New Hope this year? We certainly hope so! Before you do, however, we need all visitors to view the Forsyth County School's Mandated Reporter video. After viewing the video, you will need to complete the sign off form. We will be checking that this has been done for all visitors. Thank you for helping us keep our children safe in our schools! The video may be found here. After viewing, complete the sign off sheet here.

Permission Forms Update!

Thank you to our families who have signed the beginning of the year permission forms in Parent Portal! Right now we have 67% of our parent permission forms completed! If you have not signed these forms, please log into your Parent Portal account as soon as possible to sign them. For instructions on how to electronically sign the forms, click here. Only 1 parent needs to sign for each student.

If your child is in grades 3-5, he or she must also login to Campus Portal through Classlink and sign the form after a parent has signed. Your form will not be marked as completed until your 3-5 grade student has signed. Click here to watch a video that will guide your student through signing the form.

Update on Walkers

Thank you for your patience as we finalized our transportation routines. A few families have asked about walking their children to and from school. We will now be able to allow that with the following rules in place:

  • All students must be supervised by an adult when walking to and from school.
  • In the morning, an adult must accompany the child(ren) to the flag pole at the main entrance. A staff member will greet the students and ensure they safety enter the building.
  • In the afternoon, an adult must wait at the flag pole with their student's car rider tag. Students will be brought to the flag pole similar to how car riders are walked to cars. If you do not have your school-assigned car tag, you will be asked to go to the office for a temporary tag once our car riders have been released.
  • Crosswalks are unable to be monitored by law enforcement during pickup and dismissal. Please use caution at all times!

Please remember that if you wish to change your child to an afternoon walker, either temporarily or permanently, please complete the online form located here.

Birthday Ice Cream Purchase Information!

Please remember that no outside food or drink is allowed for birthday celebrations at New Hope. However, a great way to celebrate your child's birthday at school is to purchase an ice cream treat from our cafeteria for each student in the class!

The cost is $35 per class/$15 for SI and Pre-K classes. Birthday ice cream may not be purchased using money on your child's lunch account or by sending in checks/cash with your student. It must be purchased using Mypaymentsplus.com. Please purchase birthday ice cream at least 2 school days in advance of when you would like the class to have the ice cream.

A Note From Nurse G

Health Conditions & Forms

Dear Parent/Guardian(s):

I am thrilled to start our first year here at New Hope Elementary! We have such a great team here ready to support your child.

If your child has a health condition that may require additional support while he/she is at school please make sure all necessary healthcare action plans have been completed and brought to the school. All forms needed to help me provide excellent care to your child while he/she is at school can be found on the school nurse page of the New Hope website or by clicking here.

Please have the forms filled out completely and returned to me as soon as possible so that we may make sure your child receives the continuity of care that he/she deserves. If your student requires medication to be kept at school, a Request for Administration of Medication form (1form for each medication) must also be completed. If your child has a condition that does not fall into a specific form please use the Individual Health Care Plan. In addition, please note most forms require both a physician’s signature and the parent/guardian’s.

Please be aware that any medication brought to the school should be done by the parent/guardian. No student is permitted to carry medication to or from school.

If a condition no longer applies to your child or does not require any medical intervention at school, please provide documentation from the physician stating such. Documentation must be received before any condition(s) may be removed from our system.

I know these form(s) can feel overwhelming. I appreciate you taking time to help me provide great care to your child. I look forward to seeing your student thrive this year! Please feel free to call me, at 678-965-5101 x562106, or email me, at gsampson@forsyth.k12.ga.us, if you have any questions or concerns.
