Happy Mother's Day Pack! We are thankful for all of our moms, grandmothers, aunts, friends, and mother figures who make an impact on our Wolves! We are a stronger pack because of each of you!

8th Grade Information
Colleyville Heritage High School Schedule Adjustment Window Information:
May 15-17 is the schedule adjustment window. During this window, students may request changes to their schedule for the 23-24 school year. Changes that will be reviewed include a course change or level up/level down. Requests for changes in class periods or order of classes will not be reviewed. Changes to schedules are based on space and availability and may impact the order of a student’s schedule. To request a change, please use the CHHS Schedule Adjustment Request Form. This form will also be posted on the CHHS Counseling webpage. Schedules are unofficial until the first day of school.

The Texas Education Agency (TEA) has prepared this flyer in English and Spanish to share with our families regarding 2023 Grade 3-8 STAAR results availability. Please include the attached flyers in your upcoming parent newsletters as "Spring 2023 STAAR Reporting Timelines" and "Cronología de los informes de resultados de la prueba STAAR para la primavera de 2023".
Have your voice heard! 2023 Parent Involvement Survey for the Texas Education Agency
Does your child receive special education services? If yes, this is a reminder to complete your Special Education Parent Involvement Survey!
If your child was selected for inclusion in this year’s sample, you should have received an invitation to complete the survey at www.ParentSurveyTX.com and a PIN code. If you cannot find your PIN, or need help completing the survey, call or email Gibson at support@gibsonconsult.com or (512) 580-6008. If you did not receive an invitation, your child was not selected in this year’s sample.

Skyward Parent Portal Required Password Update
Starting May 15, all parents will need to reset their password in order to log in to Skyward Parent Portal. This is a standard security measure required by Skyward, the parent portal information system vendor used by GCISD.
Please do not change the password until May 15 or after since that is when the vendor will be requiring the new passwords.

Nurse Notes
Transfering to Another School District?
If you know your student will not be returning to GCISD next school year, please contact Ilia Kaku at ilia.kaku@gcisd.net so she may prepare school records and collect chromebooks and chargers from your students by the end of May. And you will need your child’s vaccination record for any transfer. Please print a copy from your Skyward family access before you withdraw your student from GCISD to make the enrollment process at the next school much easier. Login to Skyward (do not use the App), select your student’s name at the top if you have more than one student in the district, select “Health” at the bottom of the column on the left, then click “display options” in blue lettering at top. “Immunizations” will show in a pop up box that you can select to see and print their vaccination record. You can also take a screenshot of this record and store it in your phone for easy access in the future.
INCOMING 7TH GRADE VACCINE REQUIREMENTS - SCHOOL NURSE
Good day Wolfpack parents and guardians!
The school nurse will need an updated shot record or waiver for all incoming 7th grade students prior to the first day of school in August. The state of Texas requires documentation for Tdap and Meningococcal vaccinations for 7th grade. These two vaccines are often given at the 11th year check up by your primary care doctor. You may email me a copy of the updated vaccination record as soon as it is available to riley.heimuli@gcisd.net or fax to (817)305-4958. Please send this record as soon as possible to allow for a timely review.
If you are underinsured or do NOT have health insurance, the Tarrant County Health Department is available to provide students vaccines for a fee at the Bagsby-Williams location at 3212 Miller Ave. Fort Worth, Tx 76119 or the Watauga location at 6601 Watauga Road Suite 122 Watauga, Tx. Please call for an appointment as they will not take walk-ins. For the Bagsby-Williams location call 817-531-6738 or for the Watauga location, call 817-248-6066
If you DO have health insurance, please contact your primary care physician as your child may already have received these vaccines at an earlier appointment, and the school needs the updated record or contact CareNow, Walmart pharmacies, Walgreens or CVS as they are offering vaccinations as well. Call ahead to verify and/or to make an appointment. Remember to take a copy of your current vaccination record. You may obtain your child's record by logging into Skyward with family access and selecting “Health” on the bottom left column and “display options” then “immunizations”. You can also take a picture of this printed record and store it in your phone for easy access in the future. Please see the link below regarding information on conscientious waivers. The conscientious waiver forms may take up to 2 weeks to arrive to you and they require a notary signature so plan ahead. The waivers expire 2 years from the date of signature. The COVID-19 vaccine is not required by the State of Texas to attend school.

MEDICATION ON CAMPUS - SCHOOL NURSE
Below is a link to the medication permit. A new form is needed for each school year for any medications your child may require at school. You can print this copy and take it with you to your appointment with your child's physician or send it to your physician’s office to provide the school nurse with instructions. Controlled Medications, by State Law, require a Doctor and Parent signed Medication Permission Form with specific wording. Controlled Medications given at lunch time should say "To give at lunchtime." This allows for flexibility when schedules and lunch times change due to testing, field trips or other special days. Daily morning medications are given at home unless your 7th grade student participates in athletics and has a morning practice. Please call the school nurse to discuss and make arrangements.
Nurses are able to give a total of 10 doses for the year of non-controlled/over the counter medications with a parent signature on the medication permit, after 10 doses, we will need a doctor's signature. Please provide all medications in the original container and ensure they are not expired. School nurses do not keep stock medications.
Students may self-carry non-expired Inhalers and Epinephrine Auto-Injector medication only with a physician and parent signed medication permit (and the self carry box checked) and turned in to the nurse. Students are not allowed to self carry any over the counter (OTC) or prescription medications except the pre-approved inhaler and/or epinephrine auto-injector . Please do not send Tylenol, Ibuprofen, Tums or other OTC or controlled/prescribed medications to school with your student. This is per district policy and for the safety of all students on campus. **Please remember to label inhalers and epinephrine auto-injectors with a pharmacy label. Pharmacies will print an extra label for free. Unlabeled inhalers/Epi-pens found on campus will be properly discarded.
All other forms and authorizations (ie - Diabetes, seizure disorder, food allergy statement, etc) can be found on the GCISD website - go to Departments, pick Health Services and then on the left pick Forms and Authorizations.
Attention Incoming 9th Grade Parents:
If your student will be going to CHHS, GHS or Collegiate Academy in August, they will get a new privilege. They are allowed to self carry over the counter (OTC) medications if they have the following nonprescription medication permission form signed by the parent and student and turned into the high school/collegiate academy nurse. If you know your child will need any OTC medication including Benadryl, Ibuprofen, Tylenol, Tums, Lactaid, etc. you may fill out this form and provide it to the campus nurse. This is a parent driven form and only for high school students. Please read the nonprescription medication form carefully and follow all instructions. Any prescription medications, controlled drugs like ADD/ADHD medications MUST be kept in the nurse’s office with the appropriate medication permit with the exception of pre-approved self carry inhalers and/or EPI PENS.
Use this form for any prescription medications including self-carry inhalers and EPI PENs that your child may need during the school day. These medications require a physician and parent signature on the medication permit. Please know that school nurses do not keep stock medications. If you have any questions, you may refer to the appropriate high school nurse’s website below and/or the student handbook. For convenience, you may email a copy of the form to the high school nurse.

Student Help Desk
Help Desk - Students and staff can submit help requests to GCISD's Help Desk at any time. NOTE: If you are a parent needing help with your student’s device and your student does not know their login for Classlink, please call 817-251-5629. You will need to leave a message with your name, student's name, grade level, campus and the best phone number to reach you.
Student IDs
Thank you for helping your wolves be responsible with their student ID badges. This is a safety expectation so we can easily identify who should be at Cross Timbers. Our wolves have been doing a great job remembering their IDs given on picture day or wearing their temporary IDs for those wolves not present on picture day.
CTMS Expectations: ALL wolves wear student IDs while on the CTMS campus.
Page 6 of the Student Code of Conduct: Student ID Badges (Secondary Only) It is the expectation that all secondary students wear their student ID badges. Badges should be worn in a manner so that they are visible at all times. If students do not have an ID badge, they will be required to purchase a new badge or a temporary badge. Multiple infractions will result in further disciplinary action.
If a wolf comes to school without an ID, they will be asked to go to the front office to get a printed badge for the day. We will adhere to the following practice for missing IDs per nine weeks.

Fourth time lunch detention and parents phone call
Fifth plus times, lunch detention and pay a dollar for a printed ID
Seventh time, bring $5 for replacement ID

We will no longer be sending out daily campus notifications. Anyone needing to know the COVID case counts for the District or a specific campus, will find that information on the website.

Volunteer Background Check
Volunteer Background Check
Welcome to the Grapevine-Colleyville Independent School District. We sincerely appreciate the investment of your time as a volunteer. Whether it is volunteering with your child's classroom, your school's field day or in any capacity in connection with our partnership with Big Brothers Big Sisters, you must complete the following Volunteer Background Authorization form online.
For the protection of students and staff, all volunteer applicants must read the Volunteer Handbook (in Español) and are asked to apply online authorizing the district to obtain a criminal history background check.
Note: All volunteers are required to reapply each year.

- Planners: All wolves use a planner at CTMS. This is to assist with organizing, prioritizing, and planning. You can purchase a CTMS planner for $5 during registration or on the first day of school or use one of your choosing. Wolves use their planners daily.
- Student IDs: All wolves will be issued a temporary CTMS ID on the first day of school. On Friday, August 19 they take their school picture for their personalized IDs. The first IDs are free and replacement IDs are $5. Per the student code of conduct, IDs are to be worn daily by all CTMS students and staff.
