Causes

Most of them are caused by a blood clot, that blocks one of the coronary arteries. When blood can't reach the heart it begins to starve for oxygen., which causes it to go in to an attack. If the blockage continues long enough cells in the area will die off. The build up of plaque will also cause this stuff, which will cause it to go into attack mode.
