January Health Tips

Important Dates:
January 17th: 4K vision/hearing screens
Medications To Be Given At School
- If your child/children will need to take prescription medication during the school day a completed medication authorization form MUST BE signed by their provider, prior to any medication be given.
- If your child/children will need to take over the counter medication while at school, please fill out the medication authorization form and turn it into the school nurse.
- All medication must be brought to school by a parent or in a lock box/bag and given to the school nurse.
- If you are in need of a lock box/bag for transporting medication to school, please contact Nurse Mandy.
Medication authorization forms need to be updated every school year.
Health Plans
Health Plans are required to be updated yearly. If your child/children have had a health plan in the past an updated plan will be needed for the 2022-2023 school year.
If your child/children has a chronic health condition they may require an individualized health plan, please contact the school nurse for more information.
Health plans directly written by the doctor's office are accepted.

The program is open to all students in the district 4K through 12th grade.

If you have any questions regarding your child's immunizations or if you need a waiver form, please contact Nurse Mandy.
Notifying school when your child is absent...
If you are calling to report your child’s absence from school, please provide the following information.
1. Student name
2. Teacher
3. The reason they are missing school today.
4. Please indicate whether your child is getting tested for COVID, has tested positive for COVID, has been in close contact with someone diagnosed with COVID or if your child has any of the following symptoms:
• Cough
• Diarrhea or vomiting
• Difficulty breathing
• Fever
• Headache
• Muscle pain
• Nausea
• New loss of taste or smell
• Sore throat
