LEWERMARK HEALTH INSURANCE VACCINATION REIMBURSEMENT

Please check your email for important instructions on how to submit a claim for reimbursement of your vaccinations and T-Spot expenses.

TRAVEL SIGNATURES

If you are traveling outside of the United States please make sure page 2 of your I-20 or 1 of your DS-2019 is signed for travel.


Please follow the instructions below to get your I-20 or DS-2019 signed:


  1. Fill out the travel signature form

  2. Drop off your I-20 or DS-2019

  3. Wait for an email about when to pick up your documents
