ATTN: SPRING SPORT ATHLETES
The athletic office will begin handing out blue cards for spring sport athletes on Monday, February 14th. All students intending to participate in a sport must acquire a blue card and give it to his/her coach on or before the first day of tryouts (Monday, February 28th). The blue card indicates to your coach that you have met all the requirements for participation and that you are eligible to participate. You will NOT be able to participate in practice/tryouts without the blue card. You must get a blue card even if you've already participated in a sport already this year.
To receive a blue card the following items must be completed:
1. A current physical on file in the athletic office, must have been completed after May 1, 2021.
2. Permission to treat form on file. Can complete using the following link, https://forms.gle/176SrKnbgWzK6vWx7. Only needs to be completed once.
3. Parents and students have signed the online consent and handbook forms (completed via student verification for both student and parent).
4. Purchased an Activity Ticket from the athletic office ($70.00).
5. Have passed 4 classes in the previous semester.
See Mrs. Medeck in the athletic office with any questions.
