Curriculum Audit
Important Information:
Below is a link for you to import pictures of an artifact from your classroom onto a slide. There are no real parameters s on the artifact shared except that it should be an exemplar piece of student work from your content area(s). For example, a reader’s response, problem solving journal, science CER, or DBQ. Specials teachers may upload pictures with and explanation of the assignment. Please let me know if you have any questions. All artifacts must be uploaded by April 25th.
In addition, on May 1st, a team of auditors will complete a short observation in every classroom. Observations will take place between 8:00 and 9:00 am – each observation will last approximately 4 minutes. Below is a link to the observation schedule.
The district is still sending more information as it comes through. I will be sure to keep you updated if there are any changes.
Please complete the survey by 4-26
Northwest Independent School District leaders have contracted with an external evaluation team from Texas Association of School Administrators (TASA) to complete a Curriculum Audit. The team will be on site the week of April 30-May 3, 2018 and will be visiting campuses during that time as well as conducting interviews. However, the team will not have the opportunity to speak with everyone in the district. We would like to use the following survey to gather input from as many people as possible. Please take a few minutes to complete this survey so your opinion can be represented.
All answers will remain anonymous and survey information will only be reported in aggregate. Please complete this survey by April 26, 2018.
Thank you very much for your assistance. Your input will provide valuable information for the district.

Hello Bobcats!!

There are some awesome things happening at Lakeview!!

NISD PHA for Health Insurance Information
Personal Health Assessments - or PHAs - will begin this month for employees and spouses enrolled in a district health plan. By completing a PHA, your health care premiums will decrease from the standard rate. Dates and information about the upcoming PHAs can be found in the chart at the bottom of this email.
This year, all employees who complete the PHA process at the NISD Health and Wellness Center will have the chance to win a 55 inch LG UHD 4-K Television. Only employees currently enrolled in the health plan are eligible for a chance to win. Dependents are not eligible for the drawing.
Employees who complete the benefit enrollment sessions throughout the district in May will also be entered into the drawing for the 55 inch LG UHD 4-K Television.
Before going to have your PHA completed, please download and fill out this questionnaire. If you, your spouse or your covered dependent over the age of 18 have not been to the Northwest ISD Health and Wellness Center since Sept. 1, 2017, and will be participating in the PHA, you must also download and complete these new patient information forms.
Please remember that if you have Employee and Family or Employee and Spouse coverage, your spouse must also complete the PHA to receive the discount. Any dependents under the age of 18 do not need to participate. If you have dependents over the age of 18, they can have the PHA completed.
All PHAs must be completed by June 29, 2018 in order to receive the premium discount.
Once your PHA is completed, you will need to access your results through MyCare. Instructions to set up your MyCare account are as follows:
- Visit the mycare.texashealth.org website
- Click on "Sign Up Now" button below "New User?"
- Click the "Request Online" button below "No Activation Code?"
- Fill out the requested information and a code will be sent to you
- Go back to the "New User?" area and select "I have a code" and fill out the requested information
A special thank you to First Financial Group for providing the television for the employees who complete their PHA at the clinic rather than at any of the campus locations and for those employees who complete their enrollment at the campus enrollment sessions throughout the district.
More information about the PHA process can be found here. If you have any questions or concerns, please contact the Benefits and Risk Management Department at 817-215-0043.
