Hello Zion 6 Families

We are happy to report that the Covid-19 numbers in the county have started to drop. The mask mandate, as well as more people becoming vaccinated, has significantly helped stop the spread of the disease. The uptick in cases from July through September has created a large number of people who have short term immunity, which contributes to decreasing cases as well. Thank you to everyone for all your dedication to keeping us all safe.

State Required Compliance with Immunization and Physicals

If you haven't turned in your child's appropriate health records to your buildings nurse, your child will be excluded from school until these forms are submitted. Transportation will be suspended if it has not already been. if your student is supposed to be excluded and arrives at school, we will call for you to pick your child up. Please call your buildings nurse if you have questions.



Pre-K Health Requirements

· Certificate of Child Health Examination Form

· Immunization Records

o 3 doses of DTaP
o 3 doses of IPV
o 1 dose of MMR and Varicella
o 3 doses of HIB
o 3 doses of Hepatitis B
o Pneumococcal series or one dose after age 2

· Dental Exam (full day Pre-k only)

Kinder Health Requirements

· Certificate of Child Health Examination Form

· Immunization Records

o 4th dose of IPV and DTaP

o 2nd dose of MMR and Varicella

· Dental Exam

· Vision Exam

1st Grade Health Requirements

· Kinder Certificate of Child Health Examination Form on File

· Immunization Records

o Must have all shots listed for previous grades

2nd Grade Health Requirements

· Kinder Certificate of Child Health Examination Form on File

· Immunization Records

o Must have all shots listed for previous grades

· Dental Exam


6th Grade Health Requirements

· New Physical

· Immunization Records

· 6th graders need a Tdap and Meningococcal vaccine

ALL GRADES:

· If your child is new to the district, they have 30 days to submit paperwork from start date.

· If your child is participating in sports a sports physical is needed for 6th, 7th, and 8th grade.

· If your child’s vaccines and physical were waived last year due to remote learning, their vaccines and physical are still necessary for them to remain in school


We will accept a doctor's appointment card. This will allow your student to attend school until the date of the appointment.

Acceptable Covid Test

When your student is sent home with Covid symptoms, a negative Covid test is required.


The only acceptable Covid tests are ones administered by a doctor, provider, acute care, or approved Covid testing facility like the Lake County Health Department, CVS, or Walgreens. We require proof of negative test that has your child's name and results on it.


Home tests are NOT accepted as proof of negative COVID-19 test results.

District 6 Covid Protocols


  • 3-feet social distancing recommended within the classroom where possible.
  • Masks must be worn among all students and staff in buildings with children present anywhere in the buildings, despite vaccination status.
  • Covid testing of symptomatic students regardless of vaccination status. (If a consent to test is on file.)
  • If your child is a close contact to a Covid positive person at school while masked, they must quarantine for 10 days from last exposure to that person IF they are unvaccinated. If the contact comes from within the household they still must quarantine. Vaccinated individuals do not need to quarantine if a close contact.
  • If your child contracts Covid, please isolate them for 10 days from the start of their symptoms OR if asymptomatic, their test date. Call your school's nurse so contact tracing can be initiated.
  • If your child OR any of your household members are symptomatic with any of the below symptoms, they must stay home and either get a negative Covid test result or a doctors note to return, regardless of vaccination status. Call your school and speak to the nurse if your child has Covid like symptoms.


Covid symptoms are as follows:

o Fever over 100.4

o Cough

o Shortness of Breath

o New Loss of Taste or Smell

o Body Aches

o Sore Throat

o Vomiting

o Diarrhea
