Flu vaccinations can play an important role in our health every fall, but especially this year.

Please review the information and consider having your student participate. Help us stay safe and stay strong.

Clinic Details

Dear Parent/Guardian:

Mountainburg Public Schools will be offering combined seasonal flu/H1N1 vaccinations to all students enrolled in our school district in addition to staff members on Monday, September 28th.

We will not be offering the flu vaccine to family or community members this year.

The flu clinic will take place in two phases. It will begin at the Elementary gymnasium from 8:00 a.m. until 9:30am -10am for elementary students, elementary staff & bus drivers.

The clinic will then be moved to the Middle School gymnasium where middle school and high school students will be receiving the vaccines as well as any staff on those campuses that wish to receive the vaccine.

Each child will be bringing home a packet containing information and permission forms for the vaccinations. If you would like your child to receive this combined vaccination, please complete and return the forms by September 24th.

Please remember to complete ALL information on the forms, including insurance status. Please notice that one form is two sided. It is important to complete both sides. The FERPA form must also be completed and returned with the permission slip. Forms will be deemed incomplete and students will not receive a vaccine without a signature.


1) The mist form of the vaccine WILL NOT BE AVAILABLE at this clinic. If mist is selected on the form, your child will NOT receive the vaccine.

2) We will not be offering the flu vaccine to family or community members this year.

Dr. Debbie Atwell is serving her third year as superintendent. She and husband Roger are MHS graduates, life long residents of the community, and are owned by dogs Woodrow & Sadie.
