- If you have a 5th grade student at this campus who is not returning, their link will be found in the next section and will have a different link. This form link is only for EC-4th graders not returning.
- If you have students at other campuses who will not be returning, you will receive an e-mail from those campuses with their form links.
We would ask that you complete the form by May 15, 2020 so that we can provide further guidance on returning district devices and other items.
We understand that you already may have let us know your child will not be returning, but we need to gather additional information about your plans at this time.

Library Book Return / Dropoff
To return Lakeside library books:
- Locations: If it is NOT raining then we will place the receptacle on the sidewalk between both the office and the cafeteria loop so families can enter through either loop and have access to the bin. *In the event of rain, the return bin will be located under the covered area in the office loop.
- Please place all Lakeside library books in either a small plastic grocery, garbage bag, or a gallon ziplock bag. Please make sure that the bag is either tied or zipped up.
- There will be a large gray rolling receptacle on wheels marked Book Return
- Dates: May 18th from 8am until 11am and May 19th from 8am until 10:30am. *Pre K will be able to drop off their library books when they come to pickup their materials. Deposit all bagged books into this bin.
- This process is designed to allow families to drive thru the loops for drop-off and should not require anyone to park.
- Books will be stored over the summer and will be checked in when we return in August.
