Wi-Fi Hotspot Unfiltered Internet Access Agreement
The Chillicothe R-II School District provides access to Wi-Fi hotspot devices to Chillicothe High School students and staff. These devices allow students and staff to access wireless internet at home or on the go. However, unlike the Chromebooks provided by Chillicothe R-II School District, these hotspots have no filtering software in place.
The use of the hotspot is subject to Sprint’s Acceptable Use Policy, Privacy Policy and Terms of Use, as well as the Chillicothe R-II School District Acceptable Use Policy. Students are bound by the Chillicothe R-II School District Acceptable Use Policy, Administrative Procedure, and all other guidelines in this document and the Chillicothe R-II Chromebook Usage Policy and Handbook wherever they use a hotspot and/or a Chromebook.
Parents/guardians are responsible for monitoring what their children access while using the hotspots. Therefore, all students must have parent or guardian permission in order check out these mobile hotspots.
Use of a hotspot is subject to the terms and conditions set forth in this checkout/access agreement, and by checking out the hotspot, you have agreed to the following:
I understand that the hotspot can only be checked out by a student or staff member at Chillicothe High School. All students and staff members must have a signed Wi-Fi Hotspot Unfiltered Internet Access Agreement on file before a hotspot can be checked out.
I understand that a hotspot can be checked out based on availability. Hotspots may be checked out and picked up after the dismissal bell (after 3:00 pm) and must be returned before first hour (by 7:55 am) the following morning. The hotspot must be returned to the Chillicothe High School Library Media Specialist or Mrs. Forbes by the due date/time with all included accessories. If the hotspot is not returned on the due date, the parent/guardian will be notified. In addition, Wi-Fi service will be disconnected and the hotspot will no longer be usable. Failure to return the device will result in a $200 replacement charge. The Chillicothe R-II School District reserves the right to refuse checkout to students who abuse the hotspots or repeatedly return them late. Hotspots may be checked out to Homebound students through the Chillicothe R-II Homebound instructor for up to seven (7) days based on availability.
I understand that the hotspot runs on the Sprint network and that the speed and availability of the Wi-Fi connection will be dependent on the service area of Sprint towers. Service connection is not guaranteed in all areas.
I understand that the hotspot may NOT be returned in the bookdrop; it must be returned to the Chillicothe High School Library Media Specialist or to Mrs. Forbes. If I do not return the hotspot to the Chillicothe High School Library Media Specialist or to Mrs. Forbes in good working condition with all included accessories, I will be charged a replacement cost of $200.

Students are bound by the Chillicothe R-II School District Acceptable Use Policy, Chillicothe High School Handbook, administrative procedure, and all other guidelines in this document and the Chillicothe R-II Chromebook Usage Policy and Handbook wherever they use a hotspot and/or a Chromebook.
I understand that Chillicothe R-II School District is not responsible for any files, data, or personal information accessed, transmitted, lost or damaged while accessing the Internet via the hotspot.
