5Essentials Survey Opt Out Instructions
Dear Parent or Guardian,
We are writing to inform you about a student survey called the Illinois 5Essentials Survey that will be administered sometime after Feb. 10. The Illinois State Board of Education will implement this survey across the state. This survey is based on 20 years of research at the University of Chicago. It asks students about their experiences in school, giving school leaders critical student input to help develop school improvement plans.
Students in grades 4-8 will be required to log in using their student State ID and birth date to complete the survey. Students' identities will be kept completely confidential. Teachers or administrators will never see individual responses to survey questions and will only receive aggregated information, such as the total percentage of students who agree homework assignments help them learn the course material.
Please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c) (1) (A), you have the right to review a copy of the questions asked of your student(s). Survey questions can be found on the Illinois 5Essentials website.
If you do not want your student in Grades 4-8 to participate in the survey, we require families to electronically sign an opt-out form. The opt-out form can be found in your child’s Unified Classroom account. Please respond by Feb. 10, 2021. View this short video on how to access the form, if you need assistance.
Thank you for your cooperation.

The D103 Learning Fund Foundation invites your family to leave a legacy at D103 with the launch of the Legacy Brick Fundraiser. With the completion of the Daniel Wright construction project, the Foundation invites families to donate $100 to inscript their name on a brick pathway outside the remodeled school.
This LFF Legacy Brick Fundraiser video, narrated by Daniel Wright Principal Michelle Blackley and featuring the new school mascot, outlines the project. This is a great opportunity to bring our community together, so please consider a donation. The Learning Fund and all of our schools appreciate your generous support.
