Why have a transition meeting?
The threshold of transitioning into a formal education system is an important time in a child's life. Planning entry into school will support Kim's long-term success. Entry-to-school planning prepares the student, parents, and school for the complex transition. With a plan, we can help Kim adjust to the transition in a smooth and positive way.

Transition Process
A transition plan is designed to ease the process for children and families that are entering into school. The probability of transition success increases significantly when schools work with parents, community agencies, and healthcare professionals. The transition process will involve Kim's team to create an ultimate plan for her. We will discuss and review her strengths and needs. We will set up goals, steps, and actions. Entry-to-school planning will allow all members of Kim's team to have detailed and in-depth knowledge of the entire transition process.
School Meeting and Beyond
During the school meeting we will identify and focus on Kim's strengths and needs. We will then create Kim's transition goals. These goals must be realistic, effective, and attainable goals. Once we have agreed on her specific goals we will identify the steps and actions necessary for achieving the goals. As a group we will coordinate the plan and goals accordingly. We will create and identify realistic timelines and responsibilities. Furthermore, we will record, document, and implement the procedures and roles for the transition process. We will have open communication and have consistent meetings to ensure the plan of transitioning is ongoing and successful.
Benefits for the Future
Having a positive and successful transition from daycare-to-school will give Kim an overall positive outlook to school in general. This positive outcome will carry on throughout her school years and may also carry on to other relatable transitions. Parent's being involved with every step and decision for their child's schooling will give them knowledge, responsibility, and ownership in their child's learning overall. This will also give a positive relationship between the educator and school with the parents or family.
