JABSOM's Dr. Chien-Wen Tseng examined Medicare Part D and Cost-Sharing for HIV drugs
Published April 14, 2020, in the Journal of the American Medical Association (JAMA) is the open-access article Medicare Part D and Cost-Sharing for Antiretroviral Therapy and Preexposure Prophylaxis.
The article by JABSOM's Dr. Chien-Wen Tseng, et.al., describes their study on Medicare Part D cost-sharing of antiretroviral therapy (ART) and preexposure prophylaxis (PrEP) medicines for HIV patients.
Dr. Tseng, is the Hawaii Medical Service Association (HMSA) Endowed Chair in Health Services and Quality Research and is also a member of the US Preventive Services Task Force.
In early 2019, President Donald Trump announced a 10-year plan, Ending the HIV Epidemic: A Plan for America (EHE), the operational plan developed by agencies across the U.S. Department of Health and Human Services (HHS) to pursue that goal.
Dr. Tseng and her colleagues examined ART and PrEP costs and determined that the burden falls largely on patients and taxpayers, which is not conducive to the success of the EHE initiative.
“It seems to be apparent that achieving a 90% incidence reduction in HIV by 2030 will require legislation to manage the price of these drugs,” stated Dr. Tseng in the article.
The authors propose that Medicare Part D should be redesigned so patient out-of-pocket costs are based in stable dollar terms rather than a changing percentage of drug price. This new design should also include a ceiling for patient out-of-pocket expenses.
