Reminder: Medical Requirements
Illinois law requires all students entering 9th grade to have a completed physical exam and required immunizations on Illinois forms. Any high school student participating in a sport must also have a sports physical.
All students entering 12th grade must have 2 doses of the Meningococcal vaccine or 1 dose if the first dose was received at 16 years of age or older.
