PARENTS: It is required that proof of residency be updated yearly. Please take time to go into Parent Portal and upload appropriate documentation for proof of residency if you have not already done so. Below is a link to Parent Portal.
We are excited to be able to open South Middle to our students and their parents and guardians on August 15th for Walk About Day. Students can walk the building during this time, giving them an opportunity to locate their classes and discover the different areas of SMS. It is also a good time to visit the SMS PTO tables and sign up to become a member and purchase SMS Spirit Wear.
Team Time & Academic Arts
At the middle school level, one of our goals is to give students opportunities to try new classes. Our elective class offerings at South Middle, which we call Academic Arts, hopefully give that opportunity to students. All of our students take math, science, english, and social studies. All of our students also have an Academic Arts class that rotates every other day with health and physical education. SMS 6th graders have a class called Team Time every day. During Team Time students get time with their teachers for extra assistance, time to read, time to catch up on work, and time to participate in enrichment activities. 7th graders get one semester of Team Time that alternates every other day with an Academic Arts class and one semester of Team Time every day. Our 8th graders have Team Time that alternates every other day with an Academic Art class both semesters of the school year.
Nurse Updates
Immunizations:
All 8th-grade students are required to have a TDAP and MCV4 prior to starting the first day of school. Please provide a copy of the immunizations by emailing or faxing a copy to Nurse Natalie
South Middle is hosting an immunization clinic on August 10, 2022, from 9 am -12 pm in the South Middle School Cafeteria. If you plan on attending the immunization clinic please let Nurse Natalie know by emailing her at-- nataliespatafora@wsdr4. Walk inâs are also welcome.
Health Inventory Forms
You will be asked to complete them online before school starts. Please include all medical concerns and any changes in your child's health since last year.
Medications
All medications administered by the nurse school must have a prescription from the physician. Parent permission is also required. This includes over the counter medications such as cough syrup, eye drops and pain relievers such as ibuprofen and Tylenol. SMS has ibuprofen and Tylenol in the clinic. If your child requires liquid medication, please provide it to the nurse. For over the counter medication orders please have the physician include the dose and frequency. We are not able to accept orders that are written as, "per box orders.â
Medication orders must be obtained each school year and will expire after summer programs. All medications should be brought to the nurseâs office by a parent or trusted adult, in the original container(s). Medications cannot be accepted if they are not in the original prescription or manufacturerâs labeled bottle. The medication bottle must match the prescription orders. Do not send medications to school with your child. Please check the expiration date of the medication before bringing the medication to school.
Please click on the link below to obtain the necessary paperwork for your child if they have or need any of the following health plans. (Asthma Action Plan, Allergy and Anaphylaxis Plan, Seizure action Plan, Diabetic Medical Management Plan)

If your child sustains an injury during the school year, at home or school, please provide a letter from the physician with any restrictions that need to be implemented at school.

From Attendance:
END OF DAY DISMISSAL: At the middle school level, the office and teachers do not need to know how a student is getting home at the end of the day. However, if plans change between a parent and their child during the day, we can relay the message to the student letting them know about the change. (Ex. The student was told by their parents they would be picked up in the car rider line. The parent now wants them to ride the bus. We can pass that message on to the student.)
REMINDER: We encourage every family to check the Parent Portal and make sure that you have updated all of your contact information and the emergency contact information with names of who can check your child out early from school in the event of an emergency or family need.
EARLY CHECKOUT: You do not need to call or email the office to have your child checked out early. However, we check your ID each time prior to us calling your child out of class, so please bring your ID inside with you when you come into South Middle to check your child out.
LATE ARRIVAL: In the event your child is arriving to school late, a middle school student does not need to be walked in by their parent. They can enter the building on their own through the main entrance and sign in with us in the office. If your child was at the doctor and has a note, please have them give the note to the attendance secretary.
ABSENCES: If your child is going to miss school, please call the attendance line prior to 9:30 am and leave a message with the reason your child is absent each day they are out. If your child is going to be absent forl for several days, leave a message with dates and reason why they are not in attendance. *When your child is out of school for any reason they can email their teachers for missed work or simply check the Canvas page for their classes to stay caught up.
FOOD DELIVERY: Please know at the middle school level we do not accept food deliveries for students from restaurants or services such as Grub Hub, Uber Eats, etc.
WSD Back to School Fair
