This is a required form and must be submitted on or before February 28, 2022 | PTA Community Outreach Blood DriveOur first 2022 Vista community event is taking place soon. The Vista annual life-saving Blood Drive is taking place on Thursday, January 27th from 1pm - 7pm. Vista has successfully partnered with the American Red Cross many times to save lives. Everyone is welcome to participate (parents, grandparents, friends, family, etc). Vista and the American Cross will ensure that all safety provisions of the established Covid-19 safety plans and regulations will be firmly established and followed. To make your life-saving appointment, please visit: www.RedCrossBlood.Org. Input sponsor code: VistaSV instead of your zip code in the top righthand corner in the “find a blood drive” box. This will give you access to our specific Vista Blood Drive appointment calendar. Appointments can also be made via phone by calling: 1-800-RED-CROSS. Spots are limited. Register now. COVID Guidelines Update
Please remember
- Home kits MAY be used for students who have symptoms as a negative to return. Student must be 24 hour symptom free to return even with a negative test.
- Notify the office if your child tests positive
- Notify the office if your child is a close contact to someone who tests positive
- A longer quarantine period is required for students who are not able to isolate away from a COVID positive individual (this includes all shared spaces such as bedrooms, bathrooms, common areas)
Intent to Register for 5th & 6th grade students
For 5th grade the form indicates whether your student will:
- Remain at their elementary school for 6th grade,
- Apply for school of choice at a middle school
- Plan to move to another school outside of the district.
For 6th grade the form indicates whether your student will:
- Attend his/her neighborhood middle school for 7th grade,
- Apply for school of choice at a middle school that is not your home middle school,
- Plan to move to another school outside of the district.
