Senior Survey
All students in the PHS Class of 2019 must complete the Senior Survey.
The Senior Survey can be found on Naviance - About Me - My Surveys - Surveys Not Started
Students who plan to attend college must report the college they plan to attend so that their final transcript can be sent.
Students will not be able to pick up their graduation cap and gown until the survey is complete.
Students will be provided time during the RAMS period on May 6, 2019 to complete this survey. Students can also complete it any time on their own.
The deadline to complete the Senior Survey is May 23, 2019.
